Job Summary
The full-time Behavioral Health Care Coordinator works within a team to help clients connect with, and maintain, behavioral health/medical care and social services. This position also conducts and tracks intakes and follow-ups for the Connected Care Initiative, a project to provide integrated HIV prevention/care services for people living with, or at risk of, HIV. The Behavioral Health Care Coordinator maintains consistent communication with clients to assess their well-being and make changes to their treatment plan as needed. Work is based mainly at the Decatur Center with some days in the Duluth and Marietta locations.
Requirements
Knowledge, Skills, and Abilities
• Strong working knowledge of how social service systems (including housing) operate
• Effective communication skills – written and verbal
• Excellent organizational and time management skills
• Familiarity with professional and technical emerging knowledge
• Problem-solving skills and ability to multi-task
Minimum Qualifications
• Bachelor’s or Master’s degree in a Human Services field preferred
• At least two years of providing care coordination/case management services
• At least two years of working with individuals with behavioral health disorders
