Seeking a part time 16-18 hours a week talented care driven individual for the role of full-time Case Manager for our assisted living community. Our focus is providing our residents with the best care possible while keeping them safely in their home within our community.
Main focus is to oversee the resident’s needs and coordinate with their physicians, nurses and other healthcare providers to ensure a smooth transition and a consistent future.
The ideal candidate will have:
A Bachelor's Degree in Human Services, Case Management, Social Work or the equivalent.
Experience working with the adult or geriatric population.
Assisted Living Experience a plus
Good communication, computer and people skills.
