Polaris Healthcare & Rehabilitation Center is searching for an experienced Activities Assistant to join our TEAM!

Work times: includes weekdays and weekends may vary depending on resident needs.

Qualifications:

• High School Graduate
• Ability to write, read and speak English
• Ability to work with inidividuals who have limited capacity for self-care
• Ability to implement programs of individual and group activities based on residents' needs
• Plan and implement group and individual functions as necessary.
• Familiarity with Federal, State and local long-term-care requirements.
• Demonstrates confidence in the leadership, effective communication, and problem-solving skills.
• Be able to successfully pass all pre-employment background & drug tests.
• Previous healtcare experience or activities/recreation assistant experience a plus.

Responsibilities:

• Assist the activity director in implementing an activity program to meet each resident's needs.
• Maintain the resources needed for each resident's activity needs.
• Assist the activity director with required record keeping.

Purpose of this position: The purpose of this position is to implement an activity program in compliance with requirements to meet residents needs.
