Job Summary:
The Community Health Worker (CHW) will work under the supervision and direction of the Community Health Program Manager. The CHW will provide health related education, build trust to improve community relations, help identify and support local resident leadership and connect residents to needed health related resources. The work of the CHW will be focused on the health priorities of the residents of southern Sonoma
County based on the District’s strategic plan and core values, and an active participant in the Districts community health initiatives. The CHW will work in collaboration with the
CHW coalition in Sonoma County and other community partners, such as the Petaluma
Health Center and Petaluma Valley Hospital.

Essential Functions:
1. Provide door to door or community-based outreach to residents to assist with
COVID vaccination, complete identified screenings or assessments, offer appropriate education, conduct interviews, connect to resources, and follow-up.
2. Focuses on addressing health related social needs and acts as a bridge between community and health care and social services systems.
3. Foster trusted relationships that honor cultural norms and practices, which is essential to helping individuals get the care and resources they need in a way that reflects their preferences.
4. Assess needs based on social determinants of health and connect resident to services and resources.
5. Educates individuals about how to use the health and social service system given cultural norms; promotes and disseminates information and educational materials about public health service program services; builds health literacy and cross- cultural communication.
6. Coordinate, facilitate and/or participate in community events or informal gatherings at a variety of locations to assess needs, offer education and training based on need, and empower residents to advocate for themselves and their community, developing residential leadership.
7. Conducts interviews and provides coaching and social support in a manner that matches the residents linguistic and cultural needs.
8. Initiate contact with other health and social service providers and community resources based on need and serves as a liaison between the resident and the service provider.
9. Conduct survey as needed and collect data to assist in the determination of priorities of the Health Care District.
10. Provides translation support as explaining public health education, translating written materials, and assisting PHCD staff with translation needs as needed.
11. Attend PHCD and community meetings and act as a representative of PHCD when requested.
12. Prepare reports/documents for grant related activities.
13. Attend relevant trainings related to grant objectives.

_ _ The CHW will work in an office setting as well as doing outreach in the community., requiring travel within the community. No specific risks expected. There is a reasonably anticipated contact with blood or other potentially infectious materials that may result from the performance of an employee’s duties.

Ability to arrange tables and chairs for meetings/events. Ability to lift, load, bend, and haul relevant items to meetings/events. Able to stand for extended periods of time.
Must be able to travel to office and community events when needed.
