· Multidisciplinary services, including a Pulmonary Specialist offering Pulmonary Function Tests and Spirometry.

· Two Endocrinologists with specialties in weight loss and diabetes.

· State of the Art Location with in-house Digital Xray, ultrasound, and an Osha Regulated Lab.

· Affiliated with local Universities offering Preceptorship to future grads.

· Our care team consists of doctors, advanced practice professionals, pharmacists, care managers, quality-based coders, referral coordinators, and more.

· Team Approach offering training, collaboration, support staff, and analytics.

· Care Management and Pharmacy teams to coordinate and optimize patient care.

· Family-owned and run (non Corporate).

RESPONSIBILITIES

· Provide the best quality patient care using data informatics in a population health environment.

Required Qualifications:

• Current and unrestricted active medical license in the state of Florida.
• Demonstrate a high level of skill with interpersonal relationships and communications with colleagues and patients.
• Fully engaged in the concept of the “Integrated team-based care” model.
• Willingness and ability to learn/adapt to practice in a value-based care setting.
• Superior patient and customer service.
