Overview

Provide patient support, education and advocacy in all areas of health.

The starting hourly range for this position is $16.00 - $18.58.

Responsibilities

Perform direct patient education and advocacy:

• Use motivational-interviewing to learn about patients’ needs and utilize the process of change to identify areas for improvement.
• Provide pregnancy-options counseling (adoption and abortion) and support in a caring, non-judgmental, non-directive, non-coercive, and factual manner, allowing patient to make informed decisions.
• Provide comprehensive education and accurate information regarding methods of contraception, sexually transmitted infections, maternal and child health, and general wellness.
• Provide and explain appropriate forms to clients to ensure accurate completion and understanding, ensuring that clients with limited English proficiency receive information in the language which they best understand.

Coordinate patient care and follow-up:

• Provide information about medical services, referrals, and answer questions in a non-biased, non-judgmental, and non-directive manner.
• Schedule HED patients and maintain organized patient case load through implemented systems.
• Notify clinician of special concerns pertinent to client’s visit and provide referrals to different departments depending on needs of client such as social work and community advocacy.
• Identify opportunities to improve work flow, work processes, and patient care while working cooperatively to implement these and other quality improvements.

Implement education materials that are designed to meet the needs of specific target audiences:

• Utilize health education resources
• Prepare materials for patient distribution.

Qualifications

Education: High School Diploma/GED

Experience:

• Minimum of 2 years’ experience in a health clinic, family planning, HIV prevention, maternal health, child development, adolescent health, counseling, resource navigation, and/or experience working with health education.

Certificate/License: CPR Certified

Knowledge, Skills, and Abilities:

• Fluency in English as well as a second language preferred.
• Basic understanding of human reproduction, family planning methods, human sexuality, health prevention, wellness, and pregnancy and prenatal care.
• Ability to effectively present medical and health information related to physical health, child development, family planning, and pregnancy in order to affect health through prevention and maintenance.
• Reflective listening and strong skills in written and verbal communications.
• Knowledge of and sensitivity to the HIV/AIDS patient population and gender affirming care or an eagerness to learn about this community.
• Strong background in Health Equity, Sexual Health, STI and HIV Prevention, and experience working with populations of sexual and gender minority.
• Basic counseling and health education knowledge.
• Ability to establish and maintain cordial and professional relationships with patients, volunteers, and clinic staff.
• Ability to identify and utilize resources and referral systems.
