Case Managers needed for full-time 8am-4pm Monday through Friday at our Temporary Family Housing facilities. Looking for applicants who are dedicated and motivated to work in a complex/clinical environment dealing with challenged families who experiencing mental illness and are in need of case management. Experience working with families/public assistance is preferred but not required.
Qualifications: Bachelor's in human services OR associate's 1 years experience in human services/social work. Experience with DSS paperwork a plus!
Case management of public assistance recipients/families. Progress noting, goal plans, intake assessments, rule compliance, face to face meetings in person/over the phone, referrals to outside resources and supportive counseling.
Must be fingerprinted through the Suffolk County Department of Social Services. Fingerprinting will be paid by us.
After the successful completion of a 90-day orientation there is a generous portion of paid medical and dental benefits for full-time employees. Paid time off included for employees on schedule for more than 24 hours per week.
Must have good boundaries, reliable transportation and must be dependable during bad weather/ on holidays. Supportive management team waiting for you to join us!
