Pathways Home Health, Hospice & Palliative Care is Hiring a LVN Near Oakland, CA

Patients and their families know us for our personalized, high-quality care, delivered with empathy, kindness, and respect.TITLE: Hospice Licensed Vocational Nurse (LVN)OFFICE LOCATION: OaklandPATIENT TERRITORY: Alameda County
SCHEDULE: Full TimeSHIFT: 8:30am-5pm, with rotating weekends
POSITION SUMMARY:
The hospice LVN is responsible to provide skilled nursing care to hospice patients and their families that addresses the patient’s environmental, physical, mental, emotional, and spiritual needs. Communicates care to registered nurse (RN) case manager and other members of the Hospice Interdisciplinary Group (IDG), including patient’s physician. Provides shift care under the Medicare Hospice Benefit continuous home care level of care. Performs duties consistent with the Vocational Nurse Practice Act. AREAS OF RESPONSIBILITY:
  • Knowledgeably discusses overall history, philosophy and services of Pathways Home Health and Hospice.
  • Provides skilled nursing observation of the patient’s condition, problems, care interventions, patient and family outcomes, and evaluation of these, and documents this in the electronic record.
  • Accesses electronic device during patient visits and documents during or immediately after visits or per regulation.
  • Reports regularly to RN case manager or designee regarding care provided, and consults with the RN case manager or designee when patient condition is determined to be unstable and there is a need for change in current plan of care.
  • Supports, instructs and educates the patient, family, caregivers, and facility staff.
  • Attends hospice interdisciplinary group (IDG) meetings, educational in-service trainings, and other agency meetings as requested by manager.
  • Maintains the dignity of the dying patient and supports the patient’s and family’s unique spiritual and cultural beliefs.Provides holistic, family-centered care across treatment settings to improve the quality of life.
  • Knowledgably discusses and initiates non-pharmacologic interventions as appropriate to promote comfort, improve quality of life, and help manage emotional, spiritual, and physical distress.
  • Participates in the hospice quality assessment performance improvement program and in in-service education related to areas of practice as appropriate.
  • Meets productivity standards as established by manager.
  • When in a facility, adheres to “Pathways Standards of Care and Practice in Facilities” or designated policy document.
  • Prints out and files visit documentation in facility (RCFE or SNF) binder or chart per facility preference at the time of visit. Maintains communication with patient, family, and facility staff regarding all aspects of patient care.
  • Demonstrates awareness of and adherence to the safety, infection control and other policies of the facility.
  • Maintains and updates level of knowledge and skills required to provide quality hospice nursing care.
  • Updates and maintains medication list. Monitors medications for side effects and communicates these. Effectively instructs patient, family, and/or staff use of medications and reconciles medication records.
  • Effectively communicates with and uses the hospice prescription plan, formulary/preferred drug list, and general hospice medication coverage policies.
  • Adheres to all agency policies and procedures, including updates, as communicated in written, spoken, and electronic forms.
  • Promotes a culture of service when representing Pathways by at all times treating internal and external contacts with respect, remaining professional, communicating clearly, preventing conflict, following through on commitments, and actively listening. Positively promotes Pathways by offering assistance and information.
  • Assists in development of hospice protocols, procedures and policies as requested.
  • Demonstrates all skills of a Licensed Vocational Nurse, including those skills required by the agency through competency testing.
  • Performs other duties as assigned consistent with skills and training and the mission and goals of Pathways.

QUALIFICATIONS: 

  • California LVN license. Must have “IV Blood Withdraw” on discipline license.
  • Minimum of one year experience as an LVN within the last three years
  • Must be highly motivated, self directed, flexible, and able to work well and participate in a multidisciplinary team setting. Excellent nursing assessment, communication, and problem-solving skills.
  • California driver’s license and automobile insurance with minimum bodily injury liability coverage per agency policy.
  • Current CPR Certificate – American Heart Association “BLS Health Care Provider” requested upon renewal.
