Outreach Community Health Centers is Hiring a TCM Program Manager Near Milwaukee, WI

JOB PURPOSE AND REPORTING STRUCTURE: Under the direction of the Director of Community Services, the TCM Program Manager oversees the quality of treatment services delivered in TCM, and performs direct clinical supervision of the treatment team and the screening and admitting of new clients to TCM, in accordance with agency policies and procedures.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

  • Responsible for the day to day operations of the TCM program and supervision of all TCM case managers, peer support and clerical support.
  • Provide clinical supervision to the TCM treatment team, (one-to-one, team and side-by-side), ensuring the best possible services to clients.
  • Oversee the quality and frequency of treatment services provided to TCM clients, ensuring the meeting of the agency and contact requirements
  • Assign clients to case manager caseloads, complete intakes, initial assessments and all other paperwork related to the intakes.
  • Follow and implement the fee-for–service contract and its obligations through Milwaukee County
  • Oversee the job performance of the TCM team, coordinating with the staff on remediating job performance issues. Complete productivity reports on staff, sign off on timesheets, requested time off and other personal related activities.
  • Collaborate on the programs yearly budget and other information for the program.
  • Coordinate the development of the initial plan for services within 30 days of admission to the program. Also review and coordinate intakes if not completed by oneself.
  • Review all treatment plans for TCM clients, ensuring the meeting of agency standards. Review and sign off on other paperwork that is frequently submitted to Milwaukee County for continuation in the program.
  • Provide case management and treatment services for assigned smaller caseload. Help provide coverage to other case managers if needed.
  • Complete Discharge paperwork if client meets predetermined discharge criteria.
  • Provide continuing education to all TCM staff to improve their competency levels.
  • Track clinical supervision hours as mandated by HFS 34.
  • Complete special projects as assigned.
  • Maintain technical competency and remain current in technology and changes in the industry.
  • Complete and maintain all required paperwork, records, documents, etc.
  • Follow and comply with all safety and work rules and regulations. Maintain departmental housekeeping standards.
  • Perform on-call coverage as scheduled.
  • Continue to maintain a small case load and all the responsibilities of that individual

EDUCATION and/or EXPERIENCE: Requires master’s degree in a job related major and three to five years of related experience including 3000 hours of clinical post master’s degree supervised hours and at least one year of supervising experience. Requires LPC or LCSW licensure. 

CERTIFICATES, LICENSES, REGISTRATIONS: Requires an LPC or LCSW and a valid WI driver’s license and vehicle insurance.

LANGUAGE SKILLS: Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to prepare reports, business correspondence, and procedure manuals. Ability to effectively communicate with clients and other employees of the organization.

MATHEMATICAL SKILLS: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent.

REASONING ABILITY: Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.

OTHER SKILLS AND ABILITIES: Requires the ability to operate a pager and cell phone. Requires the ability to operate a variety of standard business machines, such as a computer and the job related software, a keyboard, calculator, telephone, FAX, photocopier, etc.
