Outreach Community Health Centers is Hiring a Clinical Patient Navigator Near Milwaukee, WI

**This position is eligible for the National Health Services Corp Loan Repayment Program for up to $50,000 for just a 2-year commitment.**
This position includes a $1,000 sign on bonus!

JOB PURPOSE AND REPORTING STRUCTURE: Under the Director of Behavioral Health Services, the Clinical Patient Navigator position is responsible for increasing patient access to care, through assessment and referral in the Walk-In setting, promoting patient engagement in behavioral health, primary care, and support services. They promote collaboration across disciplines to improve patient outcomes. The Clinical Patient Navigator will also provide direct clinical services including individual and group psychotherapies.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Assesses biopsychosocial needs of patients presenting to the Outreach Community Health Centers Walk-in Clinic
  • Creates plans of care using motivational interviewing
  • Facilitates referrals to identified services and promotes patient engagement
  • Maintains knowledge about internal and external resources to meet patient needs
  • Acts as a liaison between the service providers and advocates for patient needs
  • Provides individual as well as group psychotherapies and meets agency standards for quality and quantity of services
  • Ensures that documentation meets agency, county, state, and federal regulations
  • Completes and maintains all other required paperwork, records, and documents
  • Follows treatment and service guidelines established by Wisconsin Medicaid, Medicare, regional HMOs, private insurance companies, Milwaukee County BHD, and any other sources of reimbursement for behavioral health services
  • Provides on-call services when scheduled and offers crisis intervention when necessary
  • Maintains professional competency through continued education
  • Follows all safety and work rules and regulations and maintains departmental housekeeping standards
  • Completes other responsibilities and projects as assigned

EDUCATION and/or EXPERIENCE:

  • Requires a master's degree and a license to practice in the state of Wisconsin.
  • Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC) required (substance use credential preferred)
  • Training licenses (APSW or LPC-IT) are accepted (as well as fully licensed individuals)
