JOB PURPOSE AND REPORTING STRUCTURE: Reporting to the Behavior Health Clinic Manager, the Intake Coordinator is responsible for managing referrals to the behavioral health clinic, assisting with documentation and system monitoring required by contracts and grants.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following:

• Coordinates internal and external referrals for psychiatric and psychotherapy services in the behavioral health clinic, including scheduling, maintaining wait lists, procuring relevant medical records, communicating with internal and external providers, documenting communication in Epic EHR etc.
• Establishes and maintains provider schedule templates (Psychiatrists, APNP’s, Psychotherapists). This includes templates for other clinics within the department, i.e. School-Based Behavioral Health, Salvation Army, etc.)
• Assists with documentation required by various contracts, including Milwaukee County Behavioral Health Department, Comprehensive Community Services (CCS), Children’s Youth and Family Services (CYFS), Wrap Around and any other sources of reimbursement for behavioral health services.
• Provides administrative support (ordering office supplies, copying intake packets, completing check requests, etc.) to the department as assigned.
• Utilizes Epic EHR to document care coordination.
• Tracks patient discharges, coordinating communication with patients and providers.
• Tracks patient stipulations/commitments.
• Completes other duties as assigned.

EDUCATION and/or EXPERIENCE:  A Bachelor’s in Social Work, Psychology, Counseling or related degree is preferred. Associate’s Degree in related field will be considered.Considerations & Statement
