Oriol Therapy Services, provides therapy for patients in our skilled nursing facilities as well as home care therapy for our Oriol Home Health clients. We are looking for an experienced Physical Therapist to join our Home Health team! 

General Summary: To provide skilled physical therapy servicesin the facility or other appropriate settings consistent with professionalstandards and policies. To participatein the development of programs necessary to achieve objectives as they relateto patient care.

Qualifications:

• Graduate of an accredited Physical Therapy Program
• Minimum of a Bachelors Degree
• Licensed in the state(s) of practice

ESSENTIAL JOBFUNCTIONS:

• Completes all documentation required and in accordance with state and federal regulations and policy
• Perform patient assessments and evaluations to determine functional level and establish a therapeutic program.
• Develop a plan of care when therapy services are provided.
• Document, within time frames, observations, patient’s response to treatment, treatment programs, degree of progress and notable changes in patient condition.
• Communicate and coordinate patient care with other members of the patient’s health care team.
• Supervise physical therapy assistants in accordance with physician’s orders and accepted professional practices.
• Provide patient and family teaching related to the patient’s physical therapy program and its maintenance.
• Provide physical therapy in accordance with physician’s orders and accepted professional practices.
• Assist the patient and/family or to procure needed equipment or supplies.
• Utilize appropriate hand washing, standard precautions and personal protective equipment.
• Attend staff meetings and patients’ conferences.
• Maintains skills and education by attending continuing education and in-services programs.
• Other duties as required.
