Title:Dietitian

Brookside Multicare in Smithtown, NY has an opportunity for a Full-Time Clinical Dietitian.

Position Summary:

  • Quarterly, Annual and Significant change Assessments
  • Care planning
  • Weight monitoring
  • Meal rounds

Responsibilities Include

  • Evaluate the nutritional status of patients by reviewing the electronic medical record, the hospital discharge papers, the interdisciplinary health care team and family input

·Knowledge of therapeutic diets provided by the department, recommendations, and calculations of enteral / parenteral nutrition support.

  • Apply knowledge of nutrition screening assessment to specific patient populations to define their nutritional needs
  • Document pertinent findings in the medical record as per policy
  • Provide nutrition counseling to patients and/ or family members as indicated
  • Implement and revise nutrition care plans as needed
  • Use professional judgment to adapt, as well as apply guidelines, protocols plus professional standards of care to the changing needs of the patient
  • Evaluate the need for enteral and parenteral nutrition support, providing recommendations as appropriate

·Actively participates as a member of interdisciplinary team, attending patient rounds and meetings as required.

·Serves an educational resource to nursing and medical personnel

  • Attend care plan meetings
  • Communicating with members of the health team
  • Complete MDS documentation
  • Assist with discharge planning
  • Menu planning as needed
  • Knowledge of SIGMA Care preferred
  • Knowledge of gerimenu preferred

Requirements: Bachelor's Degree in Nutrition or related major, Registered Dietitian or RD Eligible.Customer service, good interpersonal communication skills
