The position we're recruiting for:
Main work responsibilities: Additional day-to-day tasks will be discussed during the second interview with the company’s Hiring Manager.
- Responsible for performing a varietyof casework duties, including meeting with guests, and preparingevaluations, including obtaining, analyzing, and evaluating data and developingservices plans. Responsible for making the residents aware ofthe facility's rules and regulations and providing information about theservices offered.
- Responsible for caseload and performsrecord keeping functions as required, case notes, census statisticalreports while making sure all records are maintained at a confidentiallevel.
- Refers guests to agencies and programs asnecessary and establishes a rapport and professional relationship with allstakeholders.
- Assists guests in establishing relationshipswith outside services. Assists in arranging transportation to necessaryappointments.
- Prepares a facility workshop and meeting withclients relative to goals of Social Services and needs of residents and worksclosely with other personnel assigned to residents.
- Handles emergencies as they arise and arrangesfor medical services or guidance to the medical team.
- Attend regular staffmeetings and documents progress to supervisor.
Requirements
The Background Qualifications they need- Education and/or Experience:
- HS Diploma or higher
- Preferred: Bachelor's Degree in one of the following fields: Social Work, Psychology, Education, Rehabilitation, Occupational Therapy, Counseling, Community Mental Health, Sociology, Physical or Recreational therapy. Degrees in other related areas may be considered
Experience:
- One (1) year of related human services experience required in providing direct services to clients suffering from or experiencing medical, behavioral, substance or social issues and knowledge of resources, in order to link them to a broad range of services essential to successfully living in the community
- You must have excellent interpersonal and time management skills
Preferred:
- Knowledge of Medicaid, Social Security, and other entitlements
- Bilingual in Spanish a HUGE PLUS
