Responsibilities

Provides quality patient care via the development, implementation & evaluation of individual patient care plans. Assists patients with coordination of care, obtaining authorizations, scheduling appointments, and providing advice and/or guidance. Delivers presentations regarding autoimmune disease clinic and patient groups.

Assists patients with health related issues like finding medical assistance and mobility assistance programs, performing insurance prior authorizations and assisting with any denial appeals or questions.

Assists patients with the coordination of care within OMRF and with other institutions’ social and health care providers which can include conducting outside referrals and completing various forms. Utilizes community, institutional, government, and industry resources to assist patients with finances, medications, services, and other health related needs.

Provides advice, guidance, emotional support, crisis counseling, and other assistance as needed to patients. Interviews patients and their family members/significant others to establish facts about their situation and gage impact of problems/causes on the patient’s functioning and health as part of a comprehensive psychosocial assessment.

Acts as an educational resource for patients/families about disease-oriented support. Provides case management services for autoimmune disease patients (primarily multiple sclerosis). Gives presentations regarding the autoimmune disease clinic to patient groups, other health care providers, and potential donors. Presents regular informal oral presentations of progress and results to supervisor and colleagues. Keeps abreast of changes in protocols and procedures.

Minimum Qualifications

LPN currently licensed in the State of Oklahoma with at least three (3) years experience as an LPN.

Must have knowledge of the information and techniques needed to diagnose and treat diseases, including symptoms, treatment alternatives, drug properties and interactions. Must be highly patient/customer service and detail oriented, driven, dependable, professional, flexible, industrious, creative, conscientious, and adaptive. Must possess superb communication, analytical and problem solving skills. Must have history of demonstrating the ability to maintain accurate documentation and strict confidentiality. Ability to deal professionally with diverse personalities and cultures in stressful and difficult situations. Ability to multi-task, work independently and as a team player. Ability to grow professionally through meetings, conferences, and department participation.

Preferred Qualifications

CMSC Multiple Sclerosis Certified Specialist and RN preferred.
