OHANA AZ is Hiring a Residential Case Manager Near Phoenix, AZ

Description

Case Manager Job Description:

Responsible for overall case management of DCS youth residing within Ohana group homes. Responsible for meeting with assigned youth on caseload (up to 25 youth) to build relationships, complete assessments, and help guide youth’s case toward a less restrictive setting. Completes all necessary documentation necessary for youth’s case file to remain in compliance with DCS and all CARD standards. Reports directly to Ohana’s Foster Care Program Manager and collaborates directly with group home managers. Schedules and coordinates case meetings per the needs of the youth. Provides a therapeutic approach during interactions with clients, training and meetings. Serves as a role model, servant leader, and cheerleader for the DCS youth empowering them to meet the goals they have set and to help move their case forward.

Additional Responsibilities, include but are not limited to:

  • Meet individually with assigned youth on a monthly basis for 1-on-1 check ins, goal setting, and relationship building.
  • CANS certified to complete assessment with assigned youth.
  • Complete Person Centered Plans for assigned youth within the designated time frame required.
  • Meet CARF standards for compliance and quality assurance that align.
  • Responsible for completing QRTP reports for assigned youth and submitting them to DCS prior to deadline. 
  • Responsible for completing intake of new youth entering Ohana group homes and youths discharge documents.
  • Collaborated with Teri’s Health Services to schedule appropriate appointments for youth and receive necessary medical documentation.
  • Expert in Extended Reach Software system to organize all youth documents, case notes, compliance, etc. in a timely manner.
  • Partners with the Ohana House Managers to receive and provide relevant information on progress of youth’s cases and attend appropriate youth meetings like CFT’s, court dates, IEP, therapeutic related appointments, etc.
  • Serve as Ohana’s Point of Contact for all CFT teams. 
  • Transport children as needed to relevant appointments.
  • Provides direct support to youth post crisis behaviors and collaborates with Program leadership to determine necessary services and is responsible to advocate for supportive services to the youth’s team.
  • Develops service plans and goals for each kid and create steps for them to work towards those goals (e.g., hygiene, healthy relationships, emotional regulation, getting a job, passing classes, etc) 
  • Assist managers in school enrollment of children knowing that children will most likely need IEP meetings, and serving as point of contact for the school.
  • Provides relevant information to Program Manager and Executive Director for licensing documentation including monthly reports, service plans and weekly workbook that includes the census reports, staff roster and quarterly report for Ohana Family Homes. 
  • Any other responsibilities as assigned by the Program Manager or Executive Director.

This is a full-time salaried position, Monday-Friday. Compensation depends on experience and is benefit eligible after 90 days of start date.

Requirements

Minimum Qualifications:

  • Bachelor’s degree required, minimum 1 year of experience working with youth in foster care
  • Have experience working or partnering with the Department of Child Safety
  • Case Management experience preferred
  • Demonstrates strong boundaries and shows care and concern for children in crisis
  • Excellent written and verbal communication skills
  • Possess a valid Arizona drivers’ license
  • Must maintain current First Aid, CPR, and CIT certification 
  • Be able to obtain a valid Level 1 Fingerprint Clearance Card through the Department of Public Safety and a physical examination prior to employment
  • Must be able to work in a culturally diverse environment 
  • Must have excellent organizational and time management skills
