Compensation: $17.00 per hour

Days: Monday, Weds, Fridays 8:00 AM to 4:00 PM

Job Description

  • Conduct and manage client screenings within appropriate time frame.
  • Collect and verify all collateral contacts information.
  • Collect and manage all legal information (case #’s, certificates, and letters) and inputting into appropriate systems.
  • Coordinate admission of clients with clinical team.
  • Create client files for new admits in multiple systems to ensure accuracy and continuity
  • Provide general program information and answer client questions in a professional manner.
  • Assist with coordination of referrals if the client is not appropriate for care.
  • Complete QA activities to ensure client files and admission paperwork are completed accurately, consistently.
  • Communicate and complete training/meeting notes with coworkers on QA issues that arise.
  • Maintain admission paperwork to be in compliance with program and agency needs.
  • Disperse and record quarterly surveys with clients throughout treatment episode
  • Guide new admits through admission and program process.
  • Enter in client funding and ensure accuracy.
  • Help with documentation and data entry as needed
  • Responsible for managing and closing client files upon completion of the program in multiple systems.
  • Assist program director in managing team trainings, supplies and distribution or program materials.
  • Assist in facilitating psychoeducational groups as needed.
  • Other duties as assigned.

Requirements

Licensure / Education / Prior Experience Required:

EDUCATION: Bachelors degree in related field, or two years of related employment, or program graduate recommended by staff for employment who is in pursuit of licensure:

  • Willingness and ability to pass a criminal background check with the Salt LakeCounty Jail
  • First Aid and CPR Certification (available at Odyssey House)
  • Must comply with annual TB tests, Flu and other immunizations as required
  • Pass pre-employment drug screen (for federally illegal substances) and be willing to undergo drug screenings as a condition of employment.
  • Must meet employment eligibility
  • Must be able to work variable shifts as needed

Each employee is expected to clearly understand roles and responsibilities regarding the following: Specific job

position, time management, personnel file requirements, client record system, incident reporting, mandatory

training requirements, maintaining proper client boundaries, individual rights of clients and staff.

Physical Demands of the Job: Ability to lift up to 15 lbs, driving (or adequate alternate transportation), sitting for

prolonged periods, light to moderate physical activity at times. Able to passively restrain residents without harming self.
