Oak Orchard Health is Hiring a Community Health Worker Near Albion, NY

Description

For over four decades, Oak Orchard Health has provided comprehensive, culturally aware, integrative health services to the farmworkers and their families living and working in our community. We are searching for a Community Health Worker to join the Outreach team in the Albion office. This team is passionate about connecting our entire patient population to the resources they need in order to receive the healthcare they deserve.

Requirements

  • Maintains strict confidentiality sharing information with appropriate staff, following HIPAA guidelines and Center Policies and Procedures for release of information in the various internal and external agencies with whom you are working
  • Provides written verbal translation/interpretation for Spanish speaking non English proficient patients within in the health center and within the community for health related issues, especially area migrant sites
  • Assists the target population to obtain health and secondary services
  • Assist Spanish speaking, low literacy, and elderly patient to navigate the health care sytem.
  • Serves as a liaison with professional and paraprofessional people and agencies acting as an advocate for individuals with unmet needs
  • Assists eligible patients to enroll in internal and external financial health plans I.E. Medicaid, Medicare, Child Health Plus, WIC, Sliding Fee, and Charity Care Programs either at the center or by referral to participating health plan enroller.
  • Transports referred patients and/or arranges transportation, to appointments at the Center or other agencies using the Health Center vans, when no other transportation is available and when requested in advance.
  • Travels to area where MSAWs live to promote the center and recruit patients.
  • Registers MSAWs in farmer housing setting and conducts orientation for new and returning MSAW patients
  • Acts as a liaison for OOH and DOH providers and nurses when visiting area farm worker housing locations for visits, immunizations, health education, and screenings
  • Assists patients leaving the area to find care in their new location.
  • Assists eligible patients to enroll in internal and external financial health plans I.E. Medicaid, Medicare, Child Health Plus, WIC, Sliding Fee, and Charity Care Programs either at the center or by referral to participating health plan enroller.
  • Assists patients with prescriptions renewals and refills, utilizing local pharmacy prescription programs.
  • Works with patients and community agencies to increase awareness of health promotion and disease prevention programs. Develops effective working relationships to reduce barriers in the community for at risk patients.
  • Identify any barriers patient may have such as low level reading and/or illiteracy, and work to reduce these barriers. Provides low literacy educational materials, verbal communication and/or visual education in patient’s language of choice if possible

Requirements

  • Bilingual – Spanish and English
  • Current New York State driver’s license required
  • Travel throughout our five county catchment areas and periodic travel within New York State.
  • High School Diploma or GED required
  • Education/certification in Human Services or health related field preferred (comparable experience will be considered)
