Responsibilities

The Licensed Practical Nurse is responsible for administering quality skilled nursing care to patients on an intermittent basis via development, implementation and evaluation of individual patient care plans in the home setting. This is performed in accordance with physician orders, patient plan of care, agency policy and under the direction and supervision of the RN Case Manager. This is a non-exempt position. The Licensed Practical Nurse reports to the RN Case Manager. The Licensed Practical Nurse supports and creates positive environments through a personal commitment to the Company’s vision and values.

Qualifications

• Current state licensure as a Licensed Practical Nurse in good standing in CA.
• Two (2) years progressively responsible clinical experience.
• Ability to exercise initiative, critical thinking skills, and independent judgment.
• Excellent writing, organizational, and time management skills
• Ability to work independently, multi-task, prioritize and function in a team environment.
• Must possess a professional, courteous, and customer oriented demeanor. Must be discrete, trustworthy, and possesshigh level of integrity.
