Job Description:
Job Summary:
Baptist Health Pharmacy services are committed to ensuring the optimal, safe, and effective use of medications for all patients through advanced technology and innovative application of skilled personnel that will improve clinical outcomes and community health in a value based environment. All pharmacists are responsible for drug preparation and distribution for medications at Baptist Health Louisville; evaluating, inputting and verification new medication orders; assisting providers and other health care professions by serving as a drug information resource; monitoring and managing medication regimens per protocol or per consult; and managing pharmacy inventory. Pharmacists also direct work of ancillary personnel as necessary.
Principal Duties and Responsibilities:
The following is a summary of the major functions of this individual's job. He or she may perform other duties which are not mentioned below, and specific functions may change from time to time.
1. Interprets, profiles, and dispenses medication orders for Baptist Health patients. Performs calculations as needed.
2. Supervisions production of compounded products (sterile and nonsterile, hazardous and nonhazardous) for first doses, scheduled deliveries and batched products as required.
3. Maintains knowledge of local, state and federal regulations regarding pharmacy practice as well as Joint Commission standards. Works to ensure compliance with these regulations. Performs Medication Area Inspections as assigned, contacting both Pharmacy Manager and Patient Care Unit's Manager regarding issues noted.
4. Supervises department or assigned work area operations in absence of Manager or Director including scheduling appropriate staff coverage when personnel call in sick. Coordinates, directs, and reassigns support personnel time as appropriate.
5. Initiates, documents, and participates in department activities that promote positive patient outcomes. These activities may include, but are not limited to: - Identifying medication process issues and implementing solutions - Participation in Code Blue activities - Drug Utilization Evaluations - Adverse Drug Event Reporting - Working with Inter-professional teams to optimize order sets, automated dispensing cabinet inventory, etc, medication related policies, etc.
6. Assists in orientation and training of pharmacists, technicians, students and residents. Aids in performing competencies on staff, both new hire and annual, as required by Manager or Director.
7. Participates in special projects. Examples include but are not limited to: - Identifying and implement ideas regarding methods to decrease costs - Developing new quality control procedures - Providing education to pharmacy technicians, pharmacists, pharmacy residents, and/or pharmacy students. - In-services to patients, employees, other healthcare professionals or the general public - Developed and implements staff engagement activities - Developing, reviewing, updating policies related to medication management at Baptist Health Louisville Projects must be specific, measurable, time bound and achievable. They must be approved by pharmacy Management, completed in a timely manner and of good quality. Staff should document their products.
8. Handles patient/staff complaints and troubleshoots automation issues. Reports issues for appropriate follow-up to management
9. Monitors and coordinates workflow of medication preparation and delivery, working to provide timely delivery of medications. Works with inter-professional staff on patient care units to streamline medication processes to provide optimal pharmaceutical care.
10. Is an active member of the inter-professional team on patient care units. Participates in unit based huddles and activities if available. Knowledgeable in unit's metrics and goals and works with staff on unit to help achieve goals.
11. Help perform quality assurance checks including monitoring technician tasks, ensuring daily documentation complete, etc. Fills out appropriate documentation of these activities as required.
12. Communicates with Manager, Director, and Clinical Coordinator when needed and in appropriate time frame.
13. Participates fully in the Clinical Pharmacy Program at Baptist Hospital East, including, but not limited to: - Performing interventions with medical staff to achieve optimal drug therapies for disease states - Promoting effective and efficient conversion of IV to oral therapy for appropriate medications - Assisting the Clinical Manager in gathering, trending, and analyzing Drug Utilization Evaluation data - Promoting appropriate use of anti-infective therapy - Assisting the Clinical Manager in the achievement of medication therapy goals as identified by the Pharmacy and Therapeutics Committee - Assisting the Clinical Manager in the development of patient and staff education materials and programs - Assisting the Clinical Manager in the development of medication therapy protocols based on current best practice standards - Presenting educational programs to patients and hospital staff - Designing, implementing, and monitoring appropriate medication dosing regimens - Promoting formulary therapies to the medical staff, contacting providers with alternatives and using knowledge of medication formulary, current interchanges and non-formulary medication process to make appropriate recommendations - Providing timely and accurate pharmacotherapy information to medical and hospital staff - Performing literature searches as needed/requested - Providing medication counseling to patients - Performing medication reconciliations on patients - Working with inter-professional patient care teams to help patients with transitions to care, including but not limited to helping prepare patient for discharge - Keeping current with pharmaceutical literature and guidelines, working with Clinical Manager to update current practices and procedures appropriately - Communicating clinical issues to other staff (both within pharmacy and in other departments as needed) for appropriate follow-up
14. Other tasks as assigned
Minimum Education, Training and Experience Required:
A. Graduate of an accredited College of Pharmacy with a minimum of a BS in Pharmacy with a PharmD preferred.
B. Kentucky Pharmacist Registration or eligible.
C. Hospital experience and completion of an ASHP-accredited Pharmacy Residency preferred.
D. Current Advanced Cardiovascular Life Support (ACLS) certification preferred.
