Position Title: Community Health Worker
Location: Total Health Community Clinic
Hours: Full-time, 40 hours per week, Monday - Friday, with occasional after hours availability for community events
YOUR ROLE:
Our Community Health Workers (CHW) work in a team-based patient centered care setting, partnering with the primary care team, our Transitional Programs, and community organizations to help patients navigate the healthcare system. As a CHW, you are passionate about health equity and ensuring that everyone we serve has access to the care and resources they need to improve individual lives and health outcomes.
Through relationship building and using a variety of screening tools, you will identify each person’s needs, and work to reduce barriers that may be preventing them from achieving their health and wellness goals. You will assist patients in managing appointments, assist with referrals, and seek solutions and services tailored to the needs of each patient. Above all, you will empower patients to manage their own health. This position requires a flexible, self-directed person, who is excellent at handling situations where all the details may not yet be available or determined and can search for the information that is needed.
