North Country Family Health Center Inc is Hiring a Community Health Worker Near Watertown, NY

Description

POSITION SUMMARY:

The Community Health Worker Provides care coordination (in person or via telemedicine), and works as part of a team with other Care Coordinators and the Population Health Team, Patient Service Representatives, Providers, the Family Practice Administrator, the RN Chronic Care Manager, the HIM Program Manager, and Billing to ensure smooth continuum of care for identified patients, many of which are high risk. This includes customer service, making follow-up appointments, referrals/referral follow up and documentation, telephone calls to patients, ensuring patients are able to make scheduled appointments and employing enabling services as needed, and directing inquiries; as well as documenting all activities within the Health Center’s Electronic Health Record (EHR) or other health information systems. The Community Health Worker also completes outreach services to patient to close gaps in care in an effort to reduce Emergency Department visits to the most vulnerable patients. The Community Health Worker will work closely with the Population Health Program Manager and the Quality and Value-Based Programs Department in identifying patients with open gaps in care. The Community Health Worker must be able to meet with patients where they are at; to include in the home or in the hospital. The Community Health Worker must be able to work in a fast paced environment, be self-motivated, and be able to prioritize and efficiently manage multiple assignments. The Community Health Worker also serves as the point of contact, advocate, and informational resource for patients, care team, family/caregiver(s), payers, and community resources.

QUALIFICATIONS: 

An Associates or a Bachelor’s Degree in the Human Services field with experience reducing barriers to care, patient engagement, and connecting individuals to supportive resources. Administrative skills to include knowledge of patient care, medical terminology, access, and compliance issues, preferred but not required; must be willing to learn. Has excellent interpersonal and verbal communication skills. Is skilled with computers and computer applications. Gives attention to detail and possesses problem resolution skills. Has a positive attitude and high energy, is a quick learner, is adaptable to change and is resourceful and proactive when identifying and resolving problems. Valid driver’s license and means of personal transportation.
