POSITION SUMMARY:

The Director of Social Services will provide social services to our residents which may include: Admission process, facility adjustment, psychosocial needs, resident grievances, family grievances, MDS assessments, counseling, IDCP meetings, care plans, discharge planning, facilitate discharge procedures, mass health applications, policies and procedures, federal and state compliance. 

RESPONSIBILITIES:

  • Ensures resident adjustment to rehabilitation or long-term care by performing psychological screenings, initial plan of care, introduction to services and procedures and completing all necessary paperwork including MDS.
  • Goes over Resident Rights with all new admissions.
  • Ensures admission agreement completion.
  • Collaborates with families, Business Office and Medicare/Medicaid as a liaison to assist residents with applications and qualification for sub-acute/long-term financial assistance.
  • Schedules interdisciplinary team meetings with medical staff, residents and family members in order to plan care and resolve concerns. Write reports and ensures implementation.
  • Demonstrate understanding and provide guidance and counseling to residents and their families on diverse issues ranging from grief and loss to acceptance and coping with changes in living arrangements.
  • Coordinating discharge services - planning and acquisition of appropriate follow up care including: medical equipment, nursing care and rehabilitation therapies.
  • Setting up home health services
  • Ensures compliance with State and Federal regulations for annual survey.
  • Participate in Manager on Duty program.
  • Assists with the Admissions Office duties when Coordinator is not available.
  • Continues communication with all Department Heads to ensure all resident's concerns are addressed.
  • Attend Morning Meeting and update Administrator on the day's discharges and schedule.
  • Alert through email appropriate departments all planned discharges and IDCP meetings.
  • Attend and participate in the UR meetings.
  • Attend and participate in the monthly QAPI meetings and prepare reports accordingly.
  • Assist in investigations with residents, families and/or staff.
  • Establishes and maintains cooperative relationships with community service agencies to provide social, educational, and rehabilitative services for residents.
  • Interviews new residents, compiles information, and prepares social histories containing pertinent financial, educational, and psychosocial data. Identifies ordinary/unique needs of the resident
  • Attends to all resident and family grievances.

QUALIFICATIONS/SKILLS:

  • Prior experience in Long Term Care, 3 years minimum 
  • Bachelor's Degree in Social Work, Psychology or Related Field 
  • MSW and Social Work licensure required
  • Customer service
