Patient Navigator – Lung Transplantation Program.Bring your talents to NewYork-Presbyterian, and help continue to drive the success of ourLung Transplantation Program. Here,you will be the primary care coordinator for patients who collaborates with key clinical staff to ensure comprehensive care and treatment for our pre-transplant patients.In this role your will support our patients and interdisciplinary team by coordinating appointments, providing logistical support, acts as a liaison between the patient and medical team, direct patient inquires to appropriate staff, and facilitate collection of medical records, insurance and financial information. Preferred Criteria
- Experience in a community healthcare setting
- Bachelor's degree or equivalent work experience
- Fluent in Spanish
Required Criteria
- High School diploma or GED
- Diverse and culturally sensitive customer service skills
- Microsoft Word and Excel skills
