NOAH requires all new hires to have received the first dose of a COVID-19 vaccine before their start date and be scheduled for their second dose. JOB SUMMARY
Provides direct patient care in a federally qualified health center in accordance with evidence based treatment guidelines. Physician will deliver care in a team-centered approach; interact with patients and families to keep them informed and engaged in the decisions and direction of health care; provide counseling education and resource information on wellness, prevention of illness and health maintenance; and refer patients as appropriate for specialty care services and educational training for their conditions.

JOB RESPONSIBILITIES

ESSENTIAL FUNCTIONS

Clinical Duties:

• Provide direct patient care to eligible patient at the NOAH Clinics in accordance with Community Health Center Standards and evidence based treatment guidelines.
• Maintains, monitors and ensures accurate and timely documentation of patient encounter records to meet criteria set forth by HonorHealth , NOAH, HIPAA and supporting granting agencies.
• Interacts with patients and families, school nurses, teachers, social workers and other outside agencies as appropriate. Provides counseling education and resource information on wellness, prevention of illness and health maintenance.
• Supervises student and resident education rotations at NOAH sites when mutually decided upon with clinician and CMO or designee
• Provides supervision for advanced practitioners when mutually decided upon with clinician and CMO or designee
• Interacts professionally and effectively with NOAH clinic staff and management
• Acts in a fiscally responsible manner. Works with management on program sustainment protocols. Performs other related duties as assigned or required

JOB QUALIFICATIONS
EDUCATION
Required

• Doctorate M.D. / D.O.

EXPERIENCE
Required

• Board certified or eligible in Family Medicine

Preferred

• Bilingual-Spanish

CERTIFICATIONS/LICENSURE/ETC.
Required

• Physician\PHYS - Physician AZ State Licensure
• Fingerprint Clearance Card - State have a current Fingerprint Clearance card or within seven working days after employment shall apply and be eligible for a Fingerprint Clearance Card.
• BLS

Preferred

• Board certified in Family Medicine
