Neighborhood Health Association is Hiring a Community Health Worker Near Toledo, OH

General Function:

The Goal of the Community Health Worker (CHW) of the Moms and Babies First program is to collaborate with residents utilizing the Moms and Babies First program approach to reduce racial inequalities in birth outcomes and improve the health and birth outcomes among women, children and infants residing in Lucas County.

Duties and Responsibilities:

  • Collaborate with community partners to develop and implement the Moms and Babies First program approach.
  • Identify and address social determinants of health for women and children residing in the Moms and Babies First program.
  • Propose and implement initiatives to support the goals of reducing preterm low birth weights in the targeted census tracts.
  • Perform Care Coordination, CHW and home-visit efforts to focus on increasing both early entry into prenatal care and the use of infant health care; reducing health disparities; addressing the social determinants of health; and connecting those in need to evidence-based clinical interventions.
  • Conduct assessments to determine individual needs and provide face-to-face intervention with families to improve birth outcomes, maternal health, and self-sufficiency.
  • Identify pregnant women in the “Hot Zone” of Lucas County, refer them to HUB, and enroll clients.
  • Assist mothers with prenatal care in a monthly focused group setting and connect pregnant women to prenatal care if they do not already have a provider.
  • Provide educational and community resources to remove barriers to care
  • Ensure that pregnant mothers select a medical home for baby before delivery and mom/baby attend appointment; Connect mother to a medical home if they have not already done so.
  • Coordinate care using the certified Pathways Community HUB model.
  • Perform asset analysis of “Hot Zone” and identify natural neighborhood breaks and neighborhood collaboratives, housing coalition, transportation coalition, and others to build upon asset map.
  • Report and track results on activities being performed.
  • Participate in community meetings.
  • Form professional relationships with members living in identified communities.
  • Serve as an advocate for the community.
  • Submit Project and Home Visiting reports.

Skills/Qualifications

  • High school diploma or GED.
  • Be certified as a Community Health Worker in the state of Ohio, or be willing to get certified within one (1) year of hire.
  • A valid Ohio driver's license and auto insurance with an acceptable driving record.
  • Reliable transportation and willingness to travel extensively throughout Lucas County (mileage reimbursement provided).
  • Must be sensitive to cultural, religious, and ethnic diversity.
  • Possesses a pleasant demeanor, personal maturity and emotional intelligence to be able to manage working under demanding and challenging circumstances.
  • Exhibits patience, understanding and consideration.
  • Effective organizational, communication, writing and active listening skills.
  • Ability to communicate ideas in caring for patients.
  • Follows oral and written instruction.

This is a non-exempt, hourly position

This position is non- exempt and is full-time Monday – Friday, no evenings, weekends, or holidays.
