Responsibilities
-Develop social assessment and care plan, which identifies medically related social and emotional problems and needs with realistic goals and specific actions to be taken
-Maintain grievance process and logs to positive resolution
-Participate in Utilization Review and other committees as directed and needed
-Assures that notes are informative and descriptive of services provided and of residents’ responses to services
-Prepares and completes social service documentation in timely manner
-Refer resident/family member to appropriate social service agencies when facility does not provide services or needs of resident
-Develop a social history on each resident
-Completing sections of the MDS
-Coordinating room changes
-Participating in behavior meetings
-Develops discharge plan at the point of admission to provide direction for the resident's stay

Qualifications
-BSW or MSW
-Knowledgeable of social services practices, procedures, laws, regulations and guidelines governing long-term care
-Previous long-term care experience is desirable. Past supervisory experience preferred.
