Social Service Director
Polaris Healthcare & Rehabilitation Center is searching for experienced, caring & dependable applicants to join our TEAM.
Available shift: 7am-330pm
- Experience in this field and possess quality skills to implement/care for social service needs for residents in a short/long term care facility.
- Bachelors Degree in Social Work and/or Bachelors degree in Human Services including but not limited to sociology, special education, rehabilitation counseling, and psychology with one year of supervised social work experience in a health care setting working directly with individuals.
- Able to design a social service program that meets the medically related and social and emotional needs of residents and their supportive network.
- Able to work independently, exhibit flexibility, personal integrity, and be able to work effectively with residents, personnel, families, and support agencies.
- Demonstrate tactful interpersonal communication and problem solving skills.
- Be able to successfully pass all pre-employment background & drug tests.
- Familiar with Federal and State long term care regulations and requirements.
- Assist with resident Admissions process to the facility
Purpose of this position:The purpose of this position is to provide services to meet the social and/or emotional needs that affect the residents ability to achieve their highest level of function. Participate in the development of residents comprehensive care plans, develop policies and procedures to provide social services to residents in compliance with Federal, State, and Local regulations.
