Nationwide Healthcare Services is Hiring an Intake Coordinator Near Milford, DE

Job Summary:

The Intake Coordinator is responsible for enrollment of participants into the PACE Your LIFE program, including marketing, clinical evaluation and financial assessment of potential participants.The Intake Coordinator walks potential participants through the enrollment and eligibility process from referral to enrollment.He/she reports to the Marketing Director.

Essential Duties and Responsibilities:

1.Maximizes program enrollment, implements an effective, streamlined intake process and ensures a high level of participant and caregiver satisfaction.

2.Screens potential admissions to the program to determine participant needs and financial and clinical eligibility for enrollment.Assessments are often completed in the homes of the potential enrollee.

3.Coordinates all level of care decisions with the Division of Services for Aging and Adults with Physical Disabilities.

4.Participates in the Interdisciplinary Care Team, management team and other related meetings and activities as needed.

5.Responsible for assisting in carrying out the marketing plan, meeting enrollment goals, and coordinating marketing activities with the marketing and intake team.

6.Conducts community outreach, including visits to local organizations and participation in community events.

7.Explores new marketing opportunities for the program in conjunction with the Marketing Director.

8.Completes appropriate documentation related to participant assessments as required.

9.Documents all inquiries and referrals in appropriate electronic systems and forms.Performs follow-up on all inquiries that have not yet converted to admissions.

10.Maintains statistics for internal and external marketing requirements, including State and Federal data requirements.

11.Conducts admissions counseling and tours with potential participants, families, and/or representatives.Advises participants and/or families of admission requirements as established by the program.

12.Ensures proper completion, signing, and distribution of paperwork.Able to expedite assessment and admissions process in an assertive manner and with appropriate time allowances.Coordinates admission process with Delaware Health and Social Services Office, as required.

13.Works with the PACE Medicaid Eligibility Specialist to secure financial eligibility for participants for the PACE Program.

14.Presents potential participants to the Interdisciplinary Team, identifies potential problems and seeks appropriate consult from other disciplines in determination of eligibility.

15.Assembles admissions and sales packets.

16.Participates in appropriate committees, such as the Participant Council, dealing with issues related to participant care.

17.Coordinates trial visits between the potential enrollee/family and the IDT.

18.Meets with the family and the participant on the day of admission to welcome them and introduce the program.

Qualifications:

1.Education:

a.Bachelor’s degree in nursing or social work preferred, with interest in marketing and customer service

2.Credentials/Licensure Required:

a.None

3.Experience:

a.One year of admissions experience in a long-term care setting, or community health services, with experience in clinical assessments.

b.Experience interfacing with the Delaware Department of Health and Social Services a plus

c.Marketing experience, including use of social media, preferred

d.Proficient use of Outlook, Word, Excel, and PowerPoint strongly preferred

4.Age of Patients Rendered Care:

a.Adult and geriatric patients

*** Persons who have been found guilty by a court of law of abusing, neglecting or mistreating individuals in a healthcare-related setting are ineligible for employment in this position.

Knowledge and Skills:

1.Demonstrates knowledge of and respect for the rights, dignity, and individuality of each potential enrollee and participant in all interactions.Demonstrates competency in the protection and promotion of Participant Rights.Able to act as a role model for Center staff.

2.Demonstrates knowledge of PACE Federal and State regulations, including prohibited marketing practices.

3.Demonstrates satisfactory level of interpersonal skill to interact with Center administration, community and government agencies, and Interdisciplinary Team.

4.Able to understand and to follow written and verbal directions.Able to express self adequately in oral and written communication.Able to communicate effectively with staff members, other professional staff, consultants, potential enrollees, families, and participants in an interdisciplinary care setting, and to government agencies.

5.Carries out all duties in accordance with the program mission and philosophy.

6.Maintains confidentiality of participant and program information.

7.Demonstrates honesty and integrity at all times in the care and use of participant and program property.

8.Knowledge of the emergency and disaster procedures of the program.Able to locate nearest exit, and to understand and respond to written or oral instructions in case of an emergency.

9.Sufficient mobility and strength to move freely through the building, to assure participant safety at all times, and to assist, transfer, or otherwise move participants at the Center out of danger in case of emergency.

10.Demonstrates respect for co-workers and responds to needs of participants by complying with program policies on attendance, punctuality and dress code.Able to flex hours, as needed, to meet the needs of the organization for marketing and enrollment activities.

11.Demonstrates ability to prioritize responsibilities and complete projects within allotted time.Able to respond to change productivity and to handle additional projects as delegated.

12.Able to carry out the essential functions of this job (with or without reasonable accommodation) without posing specific, current risk of substantial harm to health and safety of self and others.
