Description

Job Title: Community Living Specialist

Department: Community Living Services

Reports To: Community Living Coordinator

FLSA Status: Non-Exempt

General Purpose of the Job:

Under the general supervision of the Associate Director of Operations, with specific supervision from the Community Living Coordinator, has the primary responsibility of providing care, training, supports and supervision to Community Living residents. Models appropriate behaviors and provides on the job training to direct support staff on the shift. May assume specific responsibilities as delegated by the Community Living Coordinator for efficient and effective operations and delivery of services and supports. Provides information in accordance with Agency policy and meets established time frames.

Essential Duties and Responsibilities:

  1. Provides personal support services as identified in the Person-Centered Plan to people in his/her home. Actively engages with each resident and ensures that all programs and activities are carried out for the benefit of the person, both in the home and in the community.
  2. Encourages and supports personalization of the home and bedroom.
  3. In collaboration with each resident, develops and follows weekly menus for the home.
  4. Maintains knowledge and helps to implement person-centered goals as outlined in the Person-Centered Plan (PCP).
  5. Communicates concerns related to the residents and home operations needs to the Case Manager and/or Community Living Coordinator in a timely manner.
  6. Familiarizes self with the program file of each person supported, including history, medical, and program information.
  7. Plans and implements activities according to the preferences of and in collaboration with each person supported.
  8. Utilizes positive behavior modification techniques and other instructional strategies to enhance the progress of achieving outcomes and goals.
  9. Maintains the highest level of professionalism and is responsible for being the role model for staff; maintaining a positive approach at all times.
  10. Always knows whereabouts of people supported in the home.
  11. Supports each person to develop and maintain independence and life skills, such as mobility, personal hygiene, budgeting, and cleaning.
  12. Meets with Community Living Coordinator routinely for scheduled meetings.
  13. Is knowledgeable of routine and emergency medical procedures to include but not limited to CPR, vital signs, positioning, wheelchair usage, transfers, assistive devices, splints, dentures, and walkers.
  14. Completes and/or monitors accurate completion of required documentation for all shifts in Agency database system prior to the end of the shift, including but not limited to goal data, behavior data, seizure charts, bowel movement charts, activity logs, and incident reports.
  15. Reports any/all changes in the condition of people supported and any family concerns and complaints to the Community Living Coordinator and/or Community Living Case Manager, if the issues are of a medical nature.
  16. Reviews the Medication Administration Record prior to the end of each shift to ensure documentation is complete.
  17. Checks weekly and ensures there is a sufficient supply of medications, and changes in orders are transcribed and followed.
  18. Conducts monthly fire drills and weekly water temperature checks and completes the necessary documents.
  19. Submits all house paperwork within established time frames.
  20. Monitors and assists in maintaining safety of the home. Submits maintenance requests when safety concerns arise.
  21. Supports each person to care for and maintain his/her personal belongings and ensures these are secure and controlled by the person unless otherwise specified in the behavior plan.
  22. Assists with the spending plan and purchasing of items for the individual(s).
  23. Supports and assists residents in maintaining a clean home. Promoting independent skill building when practical in such areas as laundry, food shopping, preparing food according to a menu and special diets.
  24. In collaboration with the Case Manager and Community Living Coordinator, orients new staff and substitutes to the home including operations, programming and person-specific needs.
  25. Transports people to and from appointments and other activities.
  26. Communicates medical concerns to the nurse and supervisor within the appropriate time frame.
  27. Uses crisis intervention techniques in emergency situations when residents are in danger of injuring themselves or others.
  28. Contributes to a work and home atmosphere which is positive, enthusiastic, responsible and courteous.
  29. Reads and responds to all necessary emails in a timely fashion.
  30. Uses technology for the completion of specified job duties.
  31. Attends work regularly according to assigned work schedule and in accordance with Agency policy.
  32. Attends and participates in inservice training, staff meetings and other activities to facilitate professional development.
  33. Works cooperatively with others including all staff, supervisors, administrators, co-workers, residents served, community professionals, customers, vendors and the public.
  34. Assumes other duties, responsibilities and special projects as needed.

Qualifications


  • High School diploma or general education degree (GED); and at minimum of two years working with people with intellectual disabilities; or equivalent combination of education and experience.
  • Associate Degree or college coursework preferred.

LANGUAGESKILLS:

  • Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals.
  • Ability to write routine reports and correspondence.
  • Ability to speak effectively before groups of customers or employees of organization.

MATHEMATICALSKILLS:

  • Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.
  • Ability to compute rate, ratio, and percent and to draw and interpret bar graphs

REASONING ABILITY:

  • Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.
  • Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.

CERTIFICATES,LICENSES,REGISTRATIONS:

  • Certified Medication Technician issued by Maryland Board of Nursing.
  • Valid drivers license from State of residence.
  • Must have reliable transportation and an acceptable driving record as determined by the criteria established by the Agency's insurance carrier and Agency policy.
  • This position will require the occasional use of a reliable and safe personal vehicle to transport clients to appointments, etc.

  • Ability to interact positively with clients, coworkers, community residents, family, advocates, and Agency staff.
  • Ability to provide instruction, training, and support to individuals with intellectual disabilities.
