Social Worker | Part-Time | Hybrid

**This is a hybrid position that requires working both remotely and onsite**

We are looking for a compassionate and out-put oriented Social Worker to join our growing Nao Medical Team. This team member will be responsible in managing multiple clients and their individual mental health needs. Thus, should be patient, calm and able to manage stress. 

In summary, the duties include meeting with clients and their loved ones to discuss recent successes or setbacks to their situations, developing treatment plans for clients with mental illnesses or clients recovering from traumatic events and frequently communicating with clients to ensure their physical and mental health.

Qualifications and Skills Needed

• Bachelor’s or master’s degree in social work
• Valid and current social worker license
• Minimum of one (1) year of clinical social work experience
• Excellent listening and verbal communication skills
• Able to make psychosocial assessments and develop and implement care plans
• Able to provide after-hours service as needed
• Familiarity with ECW or EMRs
• Good organizational skills
• Multilingual is an advantage
• Knowledgeable in HIPAA

Duties and Responsibilities

• Evaluate patients (existing, new and prospective) based on their needs, limitations and desires
• Address patient concerns and goals while maintaining constant communication with the patient as well as a case manager
• Build rapport with patients and their families
• Coordinate care with other providers
• Develop care plans for patients
• Provide quick and timely resolution to patient concerns
• Follow- up with patients
• Coordinate with other team members when needed
