Mountainland Association of Governments is Hiring a Case Manager Supervisor Near Orem, UT

Job Summary: The Case Manager supervisor provides supervision to social work case managers working with adults in home and community-based settings according to County, State, and Federal regulations, policies, and procedures. The Case Manager Supervisor also carries a caseload.

Compensation: Grade 20, $66,458.00 - $102,454.00 per year. Pay will be commensurate with education and experience. Pay will be by direct deposit. Includes a full benefits package of health and dental insurance, Utah State Retirement, long-term disability, paid holidays, sick and annual time accrual, 401k contributions, and more!

FLSA: Exempt

Schedule: Full time, 40 hours per week, Monday through Thursday schedule. Regular office hours are 7:30 am to 6 pm.

Reports to: Aging & Family Services Director

Duties and Responsibilities:

  • Supervises a team of four case managers and two registered nurses in case management duties.
  • Trains new case management team hires and provides ongoing training to existing team staff.
  • Manages the workflow and caseload of the case management team.
  • Assesses the work performance of the case management team members and identifies areas that need improvement.
  • Provides mentoring and coaching to team members to improve support and performance, as needed.
  • Reports performance records and evaluations to HR and department management.
  • Monitors employee time cards.
  • Responsible for understanding, implementation, and communication of new and existing program policy and rules.
  • Works closely with State of Utah Program Managers on annual case file audits and policy.
  • Uses acceptable case management practices to complete intakes and comprehensive assessments/reassessments to screen and determine program eligibility.
  • Develops, implements, monitors, and evaluates comprehensive care plans. 
  • Creates and maintains accurate clinical and financial records for assigned clients.
  • Manages fiscal expenditures of assigned case load adjusting clients to maintain desired spending levels, as applicable. 
  • Works closely with contracted service providers to ensure quality of care of services provided. 
  • Provides advocacy on behalf of clients as appropriate.
  • Provides information services and training to groups of individuals in the community related to available resources as needed.

Supervisory Responsibilities:

The Case Manager Supervisor oversees a team of Case Managers working with aging adults in the MAG region. The case management team is comprised of four, full time SSW case managers and two registered nurses who will report directly to the Case Manager Supervisor. 

Education and Experience: 

  • Bachelor’s degree from an accredited college or university in Social Work, Gerontology, Nursing or other closely related field is required
  • Must be licensed as a Social Service Worker (SSW) or Registered Nurse (RN) in the State of Utah. Licensure must be current and active.

Physical Requirements: The Case Manager Supervisor may be exposed to illnesses or other hazards in client homes. This position requires sitting at a desk or in meetings for extended periods of time, intermittent standing and walking, must occasionally lift, carry, push, pull or otherwise move objects weighing up to 20 lbs., ability to write detailed reports, regularly drive a motor vehicle within the service area and potential exposure to illnesses and to high-stress environments and situations.
