All Metro Health Care, a Modivcare Personal Care Service, is looking for an LPNresponsible for providing quality direct patient care in accordance with the Plan of Care/Treatment. You will report to the Area Director or local Branch Manager.

You will...

• Dedicated to the provision of quality services to home care private duty clients
• Adhere to agency patient care policies and procedures, including implementing patient rights and responsibilities.
• Conduct starts of care, resumption of care, assessments, and supervisory visits/re-visits
• Administer medications and treatments as prescribed by the patient's Physician and with the State's Scope of Practice for Professional Nursing.
• Implement the Professional Plan of Care, and revise and reevaluate the patient's nursing needs.
• Maintain knowledge of the patient's condition and response to treatment. Inform the Director of Clinical Services/Clinical Supervisor/Physician and other appropriate personnel of changes in the patient's condition and needs.
• Record all clinical and progress notes daily and enter them into the patient's clinical record.
• Perform additional related duties and responsibilities as deemed necessary.

• LPN with at least 1-2 years of experience.
• 1 year of Home care experience is required.
• Valid Driver's License, Registration, Insurance, and vehicle

Pay: $30.00-$40.00/Per Diem
