MultiCulture Home Care, a Modivcare Personal Care Service, is looking for a bilingual Spanish or Creole-speaking LPN Staff Nurse to provide quality nursing care and related services in patients' homes. You will report to the local Branch Manager.

You will... 

• Provides quality nursing care and related services in the home.
• Provides those services requiring skills using the Nursing Process (Assess, Plan, Implement, and Evaluate.)
• Continually evaluates the client's nursing needs and works with the client/caregiver in developing ways to meet their needs.
• Initiates and performs teaching of the client, family, and other health team members, as appropriate, to meet identified needs and achieve goals.
• Ensure employee compliance with their schedule by conducting quarterly supervisions for personal care, home health, certified nursing assistants, and homemakers, as well as any other field staff.
• Conduct ongoing and upgrade training to employees as needed. R.N. to create HHA care plans. R.N. will supervise HHA cases quarterly and as needed.

We are excited to speak with someone with the following…

• Graduate of an approved school of practical nursing.
• 2 years experience as an LPN.
• Prior Home Care experience preferred.
• Ability to provide care in a variety of settings.
• Bilingual in English & Spanish or Creole required

Pay: $35.00-$40.00/ Hourly
