RESPONSIBILITIES

  • Oversee all services and operational activities.
  • Provide comprehensive case management that includes, but is not limited to: screening and assessing clients; creating, implementing, and monitoring individual service plans; crisis intervention; connecting clients to health, mental health, employment, housing, and other community resources.
  • Foster a professional and welcoming environment in all interactions with clients, guests, vendors, and colleagues, regardless of differing identities, backgrounds, beliefs, and choices.
  • Manage and evaluate the performance of clients.
  • Develop and maintain professional and supportive relationships with staff and subordinates.
  • Continually monitor and evaluate each client’s progression through their case management plan, and develop corrective action revisions to the plan as needed.
  • Maintain accurate documentation of service objectives and outcomes as well as other services by established guidelines.
  • Actively participate in client- and program-related meetings and training.
  • Adhere to strict boundaries and professional ethics in the care of others.
  • Ensure that the facility is clean, free of potential hazards, and safe at all times.
  • Enforce house rules and policies.

QUALIFICATIONS

  • 4-year degree in human services, or a comparable combination of education/work-related experience in a social service setting; working knowledge of case management and planning techniques.
  • Experience working with homeless individuals, youth (age 16-25), and the continuum of care in a similar environment to the population of clients we serve.
  • Provide own transportation for work-related travel as necessary.
  • Maintain a clean driving record and valid driver’s license.
  • Make sound decisions and has experience handling highly sensitive and confidential information.
  • Multi-task in a fast-paced work environment.
  • Quick to respond and work effectively in crises, including the ability to de-escalate volatile situations and apply effective conflict resolution techniques.
  • Proficient in Microsoft Word and Excel
  • Excellent written and verbal communication skills.
  • Genuine interest in helping youth.
  • Patient, inquisitive, resourceful, and flexible.
  • Relatable to the young people we serve.
  • Must be able to reach, bend, walk, stand and sit for long periods and climb stairs.
  • Must be able to lift a minimum of 30 lbs
