Description

Position Purpose (Summary)

This position oversees assigned group home. Responsible to provide direct supervision at varying times to assure that individuals are provided with services and supports in accordance with their service plans, Strive policies and regulatory standards. The Residential Supervisor will participate in direct provision of services and supports and will coordinate such while onsite. 

This position supervises staff assigned to their group home to ensure they receive orientation and on-the-job training. Responsible for hiring, disciplinary action, performance evaluation, etc., for employees working in assigned group home. Facilitates a teaching/learning environment and fosters a supportive atmosphere.

Requirements

Job Essential Functions

1.

Reliable and punctual attendance is essential; expected to be at job as scheduled each scheduled day.

2.

Oversees Residential program at assigned group home including clients, individuals and employees assigned to the group home. 

3.

Attends meetings, in-services, and trainings, on an appropriate and consistent basis. Participates in the development of in-service training modules and facilitates in-service training as needed. Participates and requests meetings such as SP/IDT team meetings as needed and appropriate.

4.

Follow up on emails, phone calls, concerns, etc., in a timely manner, preferably within two business days.

5.

Responds to GER, MANE, and other investigations in a timely manner. 

6.

Facilitates a “teaching/learning” environment and fosters a supportive atmosphere for individuals in residential programs. 

7.

Assures individuals have choices and are involved in all aspects of the daily routine.

8.

Assures that individuals receive all services and supports identified in their service plans including facilitation of Individual Service and Support Plans (ISSPs), Behavior Support Plans (BSPs) and Therapy programs and protocols, Nursing care plans and protocols, as well as other identified services and supports. 

9.

Ensures that individual supervision needs are posted and adhered to.

10.

Participates in the development and implementation of new services and supports. Participates in the provision of services and supports and coordinates provision of such while on site. 

11.

Participates in developing and updating life skills assessments and skill building programs in a timely manner.

12.

Facilitates interviewing for open positions within assigned group home.

13.

Provides facility and program orientation and on-the-job training for all assigned staff in an accurate and timely manner.

14.

Ensures all program department training is up to date and accurate for all employees in an accurate and timely manner. 

15.

Conducts employee evaluations as appropriate to meet HR requirements. Routinely observes staff performance on all shifts and provides feedback, coaching and support. 

16.

Promotes teamwork amongst staff and follows up on staff recommendations timely. 

17.

Conducts regular meetings to address staff, program, and administrative issues. Consults with Supervisor regarding issues requiring elevated attention. 

18.

Reviews GER’s and T-Logs daily. Ensures documentation is objective and professional. Completes follow up for all GER’s as appropriate.

19.

Approves timecards, mileage reimbursement and allocates overtime incurred for assigned employees in an accurate and timely manner. Submits change forms timely.

20.

Completes required organization forms, including but not limited to HR Forms, Payroll Forms, Finance and Accounting Forms, etc., and submits to appropriate department in an accurate and timely manner. 

21.

Responds to personnel concerns in a timely manner. Follows staff disciplinary policies and processes and conducts corrective action in cooperation with Human Resources. Documents personnel actions and submits documentation to HR for personnel files as required.

22.

Ensures facilities are maintained in a safe, hygienic, and satisfactory manner, including adherence to all regulatory standards. Addresses and documents efficiencies noted during safety inspections and follows up in a timely manner. 

23.

Ensures needed repairs and maintenance are completed satisfactorily and in a timely manner; and ensures furnishings and equipment are maintained in good condition and are in adequate supply on a consistent basis.

24.

Ensures that emergency drills are implemented and documented as required. Ensures emergency plans are up to date and reviewed by staff and individuals on a quarterly basis. 

25.

Maintains vehicle and facility first aid kits and confirms kits are present in department and fully stocked on an on-going basis.

26.

Serves as medication trainer/qualified manager and provides oversight to staff in medication administration. Investigates and addresses medication errors timely and in accordance with established procedures. 

27.

Confirms medication/therapeutic orders are current including, but not limited to, availability of required medications. 

28.

Ensures correct documentation of all medical/therapeutic supports. Responsible to follow all health protocols and confirms department staff follow all health protocols.

29.

Assures that individuals receive all necessary routine and acute care. Assures medical/dental/therapy appointments are scheduled and attended by individuals as needed. Assures medical/dental/therapeutic orders are facilitated in a timely manner. Assures appropriate and timely documentation of all medical care.

30.

Reviews menus to assure they are nutritionally sound. Correctly notes physician ordered diets or dietary guidelines on the house menu or develops a separate menu as appropriate.

31.

Provides training to staff with regard to physician-ordered diets and ensures that these are implemented as prescribed. Ensures that staff are provided training with regard to individual safe eating protocols and that these are adhered to on an on-going and consistent basis.

32.

Participates in department and program budget development to include monitoring program budgets and maintaining budget according to established limits. Investigates over and under expenditures and implements corrective procedures. 

33.

Assures that client money management procedures are adhered to. Reviews and approves petty cash, individual money ledgers and P-Card utilization. Reports and investigates account discrepancies. Reconciles P-Card accounts monthly. Assures that staff follow designated P-Card procedures.

34.

Develops and maintains knowledge of Strive policies, regulatory requirements and licensing standards and assures program compliance.

35.

Participates in surveys by regulatory agencies as needed. Ensures implementation of plans of correction in an accurate and timely manner.

36.

Completes and submits monitoring tools per prescribed schedule, addressing any noted concerns and documenting actions taken. Assures that concerns noted through case management or organizational monitoring are addressed in an accurate and timely manner.

37.

Assures that individuals have available to them any prescribed and/or needed adaptive equipment or assistive technology. Assures that individual adaptive equipment is kept clean and maintained in good repair. Notifies therapist or otherwise assures facilitation of repairs to adaptive equipment when problems are noted. Assures that Residential environments are barrier-free to the maximum degree possible. 

38.

Participates in on-call rotation. 

39.

Responds to all emergency situations as appropriate and in a timely manner.

40.

Acts as backup for DSP positions as needed to cover open shifts.

41.

Other job-related duties as assigned.

Education (must be required for job function), License, Certification and Experience

Education

High School Graduate or Equivalent preferred.

Minimum of four years’ work experience with individuals with disabilities required.

OSHA Category Classification

JOB CATEGORY II

Tasks that involve no usual exposure to blood, body fluids, or tissues. However, because of the nature of the work, the performance of Category I tasks may be necessary. The normal work routine involves no exposure to blood, body fluids, or tissues, but exposure or potential exposure may result as a condition of employment. Appropriate protective equipment (i.e., gloves, masks, gowns, etc.) are readily available as specified in each procedure. Personnel performing Category II tasks need not be wearing protective equipment, but they should be prepared to put on such protective garb on short notice.

FIRE PROCEDURE

All Strive staff personnel have the responsibility to know and execute proficiently the requirements outlined in the Strive Fire Policy. Each employee will take the time to become knowledgeable of the fire procedures of the area of our facilities in which they are employed. Further, each employee will fully participate in all fire drills while on duty and will attend the fire extinguisher practices and fire in-services presented.

COMPLIANCE PROGRAM

Strive has a voluntary compliance program to identify and prevent any possible fraud and abuse of the Federal and State health care programs, specifically Medicare and Medicaid. Employees will be required to attend mandatory in-services about the compliance program. Employees providing direct care need to comply with ethical standards of practice. Any violations of the Federal and State health care programs may result in disciplinary action, such as termination and reporting of criminal activity.
