This position is responsible for planning, developing, implementing, coordinating and evaluating palliative care services in collaboration with the Sacramento area Mercy Hospitals and Dignity Health, in a manner consistent with the Mission and Philosophy of Dignity Health. Included in this responsibility is developing and maintaining strong relationships with physicians and hospital staff to deliver palliative, comfort care to seriously ill and dying patients. In addition, this position is responsible to collaborate with related healthcare practitioners, the patients and their significant others in assisting with admissions of appropriate patients to hospice service. This position is responsible for the enhancement and promotion of palliative care in the hospital, assuring educational opportunities; standards and program development; quality improvement activities; consultation and care management.
