Candidates must have previous OASIS experience to include: SOC, ROC, Recert and discharge.
Role Summary:
- The role focuses on planning, organizing, and delivering home care services.
- Responsibilities include assessing patient needs, developing and adjusting care plans, administering treatments, providing direct patient care, and counseling patients and families.
- Conduct initial and regular assessments of patients.
- Develop and revise care plans based on patient needs.
- Administer medication and treatments as prescribed.
- Offer guidance and counseling on nursing and related needs.
- Lead discharge planning and implementation.
- Act as a Case Manager for assigned caseload.
- Associate's or Bachelor’s degree in Nursing.
- 1-2 years of acute care experience; home health experience preferred.
- Active RN license in Florida.
- Current BLS certification.
- Valid driver's license and insured, reliable vehicle.
- Strong communication, problem-solving, and nursing skills.
- Physical ability to perform job duties.
