Who We Want

As we continue to serve more families as they transition into skilled nursing facilities, we are seeking someone to help manage our growing case load. This individual has a passion for efficiency and transparency. Our ideal Case Manager is a master at prioritizing daily tasks and has the ability to work independently as well as in a team. The Case Manager will be an excellent problem solver; finding unique solutions for our clients and case workers. The ideal candidate should be adaptable and maintain accountability for their assigned cases. They are proficient in technologies such as Microsoft Office, and quickly adapt to new technology. Most importantly, they strive for excellence, and their genuine care for others shines through.

What You’ll Do

As a Medicaid Case Manager, you will manage a small case load as well as manage a team of case workers who will have their own case loads. In this position, the ideal candidate will be able to take ownership and responsibility of the cases assigned to them from start to finish, as well as assist the Case Workers with their cases. It is important for the Case Manager to have a strong track record of caseload management, comfortable with calculating assets for eligibility limits, and working as part of a close-knit team. The Case Workers are responsible for gathering necessary documents needed by Medicaid in order to file an application for our clients. The Case Manager will keep all cases on track and progressing forward. This position requires strong organization and prioritizing skills.

You will maintain a high level of personal accountability by providing recurrent updates to our management team and client facility business office managers. This will be an in-office position, Monday through Friday 9-6 with an hour off for lunch.

Qualifications

Preferred:

  • 3 years in Case Management (huge plus if you have Medicaid experience, but not required)
  • Ability to occasionally work outside of normal work hours

Required:

  • Project/Case Management background
  • Higher education - minimum Associates Degree
  • Excellent verbal communication skills
  • Ability to effectively prioritize tasks
  • Financial accumen, ability to look at bank statements and other financial documents and understand what you are looking at
  • Responsive
  • Strong interpersonal skills - empathetic and pleasant
