SCHEDULE:

- No Evenings
- No Weekends
- Minimal On-Call (phone triage only)

COMPANY PROFILE: 
This organization is a Medicare Advantage Institutional Special Needs Health Plan (ISNP) for long-term nursing home residents. The plan covers all medically necessary and preventive services covered under Medicare Parts A, B, and D. They are strongly focused on improving their members' physical, mental, and social well-being through personalized clinical care and therapies. They are currently hiring due to growth.

POSITION DESCRIPTION: 
The provider is responsible for managing a panel of long-term SNF residents, focusing on treating patients in place in collaboration with a primary care team.

The provider conducts comprehensive assessments, develops and delivers individualized plans of care, attends care team meetings, communicates with family members / caregivers, and ensures that the care team understands the patients’ goals and needs of care.

Specific responsibilities include, but are not limited to:

- assess and plan healthcare that is individualized to the patient
- review and interpret medical, surgical, and social histories
- perform screening procedures
- interpret laboratory and other diagnostic data and recommend treatments / actions
- reconcile medications
- prescribe and/or perform various treatment modalities
- perform direct care management support
- educate care teams on patient needs and early identification of changes in condition
- work collaboratively with physicians, care coordinators, facility staff, family members, etc.

The practice uses a mobile-based EHR.

The clinical work is autonomous, with one-on-one patient visits. This is also a team-oriented position with meetings, conference calls, etc. with members of the care team.

REQUIREMENTS:
- Virginia NP license
- DEA
- 1 year advanced practice experience
- 1 year geriatric experience as an advanced practice provider
- some previous experience in SNFs, LTC facilities, etc.
