Purpose of Your- REQUIRES a ND CNA license, 

Job Position: The primary purpose of your job position is to provide your assigned residents with assistance in meeting scheduled needs as outlined in the care plan/service plan and in assisting with unscheduled needs when required according to established facility policies and procedures and as may be directed by your supervisor. Caregiver requires ND CNA License.

Delegation of Authority: As a Caregiver you are delegated the administrative authority, responsibility, and accountability necessary for carrying out your assigned duties.

Performance Evaluation Risk Exposure Rating 1 - 5 You will be evaluated based on job knowledge, performance, coordination of duties, equipment and supply use, and safety and sanitation practices. Additional general observations will include appearance, attendance, education/in-service, initiative, judgment, and teamwork.

A criteria-based performance evaluation has been made a part of this job description. Ratings are based on a rating of 1 - 5. 1= does not meet the standard; 2 = meets the standard but needs improvement; 3 = meets the standard; 4 = above average performance; 5 = exceeds the standard because of exceptional performance.

MAJOR DUTIES AND RESPONSIBILITIES COMMENTS / GOALS FOR IMPROVEMENT DOCUMENTATION

Consistently records entries on all forms. ADL forms completed according to required services, MAR completed accurately and all blanks completed, care/service plan reflects resident current condition. All exceptions or problems with residents are recorded on 24 hour report sheet. 3

ADMISSION, TRANSFER & DISCHARGE OF RESIDENT

Greet residents in a friendly and welcoming manner. Introduce to residents and staff. Orientate resident and family to community routines; i.e.: fire evacuation procedures, meal times, etc. Obtain admit height, weight, vital signs. Assist resident with unpacking/packing belongings. 1

MEDICATION ASSISTANCE

Obtains medication certification and follows all applicable state rules and laws for medication assistance. 2

Assists residents with medication administration and administers medications according to facility policies and procedures and established standards of practice. 1

PERSONAL CARE

Assist resident with ADLs such as bathing, dressing, grooming, mouth care, toileting, etc. as outlined in service/care plan. 1

Perform nursing care as delegated by the licensed nurse, including, taking and recording blood pressure, pulse, respirations, height and weight. Report any changes in resident’s condition including skin breakdown, behaviors, etc. 1

INSTRUMENTAL ACTIVITIES OF DAILY LIVING

Assist resident in meal preparation, housekeeping and cleaning of residence and laundry. Also assist resident in use of telephone and other communication needs. 1

MONTESSORI DEMENTIA PROGRAMMING

Becomes certified and maintains certification in Montessori Programming. 3

Implements concepts of Montessori programming in activities and individualized service/care plan for each resident. 1

Performs Montessori Based Dementia Programming with residents in all service programs. 1

Assists in developing and implementing Montessori Based activity program and activities. 1

Assists residents with social and Montessori Based Programming and activities i.e., baking, crafts, etc. Leads resident activity groups i.e., exercise, manicures, etc. 1

APPEARANCE

Wear appropriate clothing as described in the Dress Code policy. 3

Maintain clean clothing/uniform and shoes. 3

Wear name tag while on duty. 3

COMMUNICATION

Identify and requisition supplies as needed. 3

Seek clarification of assignments and ask questions when needed. 3

Volunteer assistance to co-workers as time permits or as directed. 3

Treat co-workers with courtesy, respect and in a supportive manner at all times. 3

Resolve interpersonal conflict with co-workers directly and privately and seeks supervisor assistance as necessary. 3

Interact with residents and maintain resident-centered conversations when participants are present. 3

Maintain an environment for residents, families and guests that is customer friendly and that values and respects the uniqueness of each individual. 3

EDUCATION/TRAINING

Attends department meetings, all staff meetings, mandatory in-services and training. Understands and uses Universal Precautions, OSHA Blood borne Pathogen Standard and follows established infection control, hazardous communication and all safety rules. 2

INFECTION CONTROL

Demonstrates knowledge of infection control practices by compliance with established policies and procedures. 3

Washes hands when gloves are removed and when soiled. Uses hand sanitizer when appropriate. 3

Uses universal precautions according to facility policy including using Personal Protective Equipment and Blood and Body Fluid spill kit when indicated. 2

Proper disposal of waste products. 1

MAINTAINS SAFE/SANITARY ENVIRONMENT

Uses proper body mechanics when performing tasks. 2

Reports hazardous conditions or equipment to manager immediately. 3

Proper and safe handling of equipment. 2

Follows emergency safety policies and procedures (fire, evacuation, elopement, etc.). 2

Reports accidents and incidents immediately to manager and completes incident report. 3

Follow established safety precautions and safe operating procedures in the performance of all duties. 2

RESIDENT RIGHTS

Address residents by preferred name and avoid labels such as sweetie, dear or honey. 3

Discuss resident status in private. 3

Maintain confidentiality of resident conversations unless doing so would jeopardize resident well-being. 3

Treat residents courteously, politely and in a physically appropriate manner. 3

Maintain the confidentiality of all resident information. Uses discretion when dealing with sensitive issues. 3

Respond to family/resident complaints consistent within facility policy. 3

Knowledgeable about resident rights and respects rights of residents at all times. 3

RESIDENT SAFETY/CONCERNS

Documents resident behaviors on the Resident Care & Concern Form and reports to manager. 3

Reports emergent resident concerns to your manager immediately. Documents any non-emergent concerns on the Health & Wellness Concern Form. 3

Working Conditions: · Works throughout the resident areas (i.e., med rooms, offices, resident condominiums, common spaces, health areas. etc.) · Sits, stands, bends, lifts, walks and moves intermittently during working hours. · Subject to frequent interruptions. Involved with residents, personnel, visitors, government agencies/personnel, etc. under all conditions and circumstances. · Is subject to falls, burns from equipment, odors, etc. throughout the work day. · Is subject to hostile and emotionally upset residents, family members, personnel and visitors. · Works beyond normal working hours, on weekends, holidays, and in other positions temporarily as necessary. · Is subject to exposure to infectious waste, diseases, conditions, etc., including the HIV, Hepatitis B (HBV) and Hepatitis C (HCV) viruses. · Communicates with other department personnel.

Qualifications: · Must possess, as a minimum, an 8th grade education. · Must be willing to become a Certified Medication Aide in accordance with the laws of this state. · Must be certified as a Nurse Assistant or become a UAP on the NDBON registry.
