POSITION SUMMARY:

Provides basic patient care including, but not limited to: age specific assistance with ADL’s, assists physicians and nurses with procedures, assist with whirlpool treatments, changing bed linens, patient escort and monitoring’ and any other therapeutic measures that result in a higher level of patient comfort. Must be able to support the association and hospital mission, goals and objectives. Reports to the Inpatient Manager.

PRINCIPAL DUTIES AND RESPONSIBILITIES:

  • Assists with admission, discharge, transport, and transfer of patients.
  • Prepares patients for and assists with examinations, tests, and special procedures.
  • Advises and assists patients in handling of valuables and personal belongings per hospital policy and assists in gathering patients belongings during transfer or discharge.
  • Provides basic patient care, including hygiene, preventive skin care, incontinence care, feeding, and mobility, range of motion and application of safety measures to all patients including those in isolation.
  • Understands the developmental needs of all patients throughout the continuum of life. Is able to assess and adjust care of specific patients based on those needs as directed by the charge nurse or supervisor.
  • Obtains vital signs; temperature, blood pressure, pulse, respirations, pulse oximetry, weight (including use of bed scale and wheelchair scale when necessary), height, head and girth measurements. Documents every shift on PCT task checklist or proper forms. Recognizes and notifies nurse of any abnormal measures.
  • Monitors for, identifies, and reports changes in patient status to the RN.
  • Performs patient treatments as directed. These may include but are not limited to: application of heat or cold, non-sterile care, post-mortem care, care of urinary catheters, and oral suctioning.
  • Performs a variety of technical tasks, which may include: collecting, preparing and transporting specimens to the laboratory, removal of urinary catheters, setting up the delivery table, and performance of accucheck blood sugars. Documents and reports results to nurse.
  • Initiates CPR and other emergency measures as required. Adheres to established emergency and disaster procedures.
  • Assists in the maintenance of unit supplies and nourishments.
  • Maintains cleanliness in patient rooms and work areas and safety of a variety of equipment, reports any hazardous equipment and seeks repair when necessary.
  • Assists in maintaining a safe environment for all patients, including 1:1 monitoring via monitor is the primary responsibility following patient restrictions ordered by the provider, de-escalation activities/Code Gray, log/notifying security of visitor screening.
  • Assists with the application and removal of restraints and instructed. Monitors patients in the restraints or on suicide precautions per protocol.
  • Advise and assist patients in the proper handing of valuable and personal belongings and assists in gathering belongings to a secured patient/visitor locker during admission, transfer or discharge.
  • Maintains confidentiality concerning patient care information and chart contents.
  • Performs computer entry (i.e., lab, x-ray, schedule appointments, and other data as necessary).
  • Maintains documentation and assists with patient paperwork according to established policy.
  • Participates in unit or hospital quality improvement projects and attends staff meetings and job related training as required.
  • Collaborates with personnel in nursing and supportive services, with other members of the health care team, patients and family members to promote delivery of patient care.
  • Uses effective communication skills in working with health care team, patients, and families using the MHC customer service model. Notifies nursing when escalation behavior noted. Maintains a pleasant and helpful attitude at all times, greets visitors and answers telephone appropriately.
  • Treats all patients with dignity and respect ensuring that they are sensitive to the patients comfort and privacy.
  • May act as interpreter for patients and medical staff in order to clarify areas of misunderstanding, assist medical staff in securing adequate and complete medical history and assist patient in understanding the illness and proposed treatment.
  • Performs secretarial duties related to clinical care as delegated by the RN such as (but not limited to): answering the phones, completing requisitions, scheduling tests and appointments, filing reports, ordering supplies and equipment, managing patients medical records. Etc.
  • Is self-directed and maintains current licensure. Takes advantage of on-going training opportunities.
  • Supervises and directs work of PCT I’s in collaboration with nurses.
Job Qualifications:

MINIMUM REQUIREMENTS:

  • Heartsaver CPR, complete AVADE training I, II, III or must obtain within 6 months of hire.
  • Must successfully complete a medical terminology course within one year of hire.
  • Ability to lift up to 50 pounds and spend long periods standing and walking. Maybe be exposed to communicable diseases and hazardous materials. Must be able to pass the core competencies assigned to the unit annually.
  • Must have the desire and ability to work with individuals under great stress.
  • Must be willing to rotate shifts and be floated to other hospital locations as needs occur such as ambulatory clinic and inpatient.
  • For PCT II – must maintain some formal health-training certificate beyond ETT, i.e. certified nursing assistance certification (CNA), emergency medical technician (EMT II), or CHAP.
