POSITION SUMMARY

Responsible for providing administrative support for the Medical Providers and the assigned Care Team. Responsible for assisting patients with questions, appointments, referrals, etc. Reports to the Outpatient Services Manager.

PRINCIPAL DUTIES AND RESPONSIBILITIES

  • Carries out reception duties including receiving, screening, and forwarding incoming phone calls, and prioritizing and passing on information.
  • Performs clerical duties as required to support assigned Care Team, including typing letters, memos, minutes, reports, faxing and copying.
  • Maintains files in a secure and organized manner.
  • Compiles and inputs data into computer system as directed by the Care Team.
  • Notifies patients of normal labs only when directed to do so by provider or nurse.
  • Requests charts from medical records and assists in Chart Reviews.
  • Copy information and distribute as indicated by Medical Providers and Care Team.
  • Assists with Care Team Projects as assigned including patient databases and pending follow-up.
  • Assists with the referral process, including patient travel, in collaboration with Specialty and Care Team staff as directed.
  • Assists Care Team with patient letters and travel worksheets as directed.
  • Participates in departmental and organizational performance improvement activities.
  • Treats all patients and staff with dignity and respect.
  • Schedule appointments at MHC as directed by the Care Team.
  • Proficient in use in Cerner or current Electronic Health Record.
  • Knowledgeable of appropriate information from the patient record that should be included with referrals.
  • Manage assigned queues.
  • Perform online cardiac event monitor registrations when ordered by providers.
  • Assistant providers and nurses by pulling reports when needed.
  • Works, prints, and forwards appropriate teams the AFHCAN queues as assigned.
  • Collaborates with appropriate case manager concerning problems in referring the patient.
  • Assists as needed in making follow up appointments or getting request to the schedulers.
  • Assists the Telerad xrays for radiology as directed by the nurse or provider.
  • Provides good telephone and customer service skills.
Job Qualifications:

MINIMUM REQUIREMENTS

Case Management Support I: High school diploma or equivalent plus 3-6 months previous health service experience preferred. Must be willing to complete Medical Terminology course and receive certification within 12 months of hire. Knowledge of EHR and RPMS system preferred. Ability to utilize various computer programs and type well enough to carry out job duties efficiently. Must be able to maintain strict confidentiality at all times. Requires good interpersonal and communication skills. Requires strong organizational skills and ability to maintain file systems. Requires excellent customer service skills and telephone etiquette. CPR/BLS required. Must obtain CPR/BLS within 6 months of hire date.

Case Management Support II: CMA or CHA III, IV or CHP certification; or CNA or EMT I certification or higher plus one year of health service experience. Familiarity with Medical terminology and RPMS or EHR system required. Certification as a CNA; CHP III, IV or CHP; CMA; or EMT I required. Ability to utilize various computer programs and type well enough to carry out job duties efficiently. Must be able to maintain strict confidentiality at all times. Requires good interpersonal and communication skills. Requires strong organizational skills and ability to maintain file systems. Requires excellent customer service skills and telephone etiquette. CPR/BLS certification required. Must obtain CPR/BLS certification within 6 months of hire.
