Hello! Thank you for your interest in joining the Wellspring team as an Assistant Director of Nursing and Infection Control Nurse. As a statewide nonprofit serving seniors, families and children in need of support, we take a whole-person approach to our organization and delivery of our services — because we believe we must find the strengths in people and build on these for lasting change to occur.

As an Assistant Director of Nursing and Infection Control Nurse with the Livonia Skilled Nursing and Rehab Campus at Wellspring, you’ll be part of a team of nurses, CNAs and senior support staff committed to helping seniors and families in Livonia. In this position, you'll be directly supporting your community in Wayne County to provide services to seniors.

In this position, you must be currently licensed by the State of Michigan Board of Nursing as a Registered Nurse and have successfully completed an accredited Registered Nursing program; BSN preferred, RN license required. Two years’ nursing experience with current long term or sub-acute care, medical, surgical, or critical care experience preferred. An Infection Control Certification is required (or willingness to obtain within 3 months of hire).
As an Assistant Director of Nursing, you will:

  • Ensure an environment which encourages each resident’s ability to maintain or attain the highest level of independence and physical, mental, and psychosocial well-being
  • Maintain the comfort, privacy, and personal dignity of each resident in the delivery of services to them Interact with residents in a manner that displays warmth
  • Support the DON and Wellspring management in clinical and operational needs
  • Survey and direct infection control practice of all departments including :Track and document every resident with nosocomial infections, track community acquired infections, map and track infections geographically and related to contacts, collaborate with DCS and Medical Director to problem-solve and prevent transmission of infections of residents and employees. High level focus on VRE, MRSA & C-difficile, provide education as needed to ensure control of transmission and appropriate treatment of infections
  • Work with Medical Director and physicians on over use and proper dosing of antibiotic therapies.
  • File all OSHA reports RT blood borne exposures.
  • Oversee TB exposure program for residents, staff and volunteers and “FIT” process
  • Coordinate the IV Team to ensure continuity of resident centered care
  • Lead orientation process for new employees
  • Oversee and ensure all required initial and annual inoculations for residents, employees and volunteers are tracked, completed and documented
  • Assist with development of skills fairs and other approaches to staff development and education
  • Ensure that yearly education is completed as required by regulations for all employees
  • Assist DON and other nursing department management staff with researching best
    practice and developing educational presentations on specific approaches needed to respond to resident care requirements
  • Evaluate outcome of care and nursing judgments on a daily basis to assure high quality care
  • Able to communicate with staff and monitor for any problems; follow up with DON if necessary and proceed with disciplinary steps as needed per policy
  • Complete evaluations for nursing staff on the unit as directed
  • Assure the aides are completing walking rounds and report to each other at shift change; assure that the nurse gives a thorough report to the aides; assure that the off going nurse gives a thorough report to oncoming nurse
  • Review EHR periodically to assure proper and timely documentation being done
  • Assure documentation reflects the resident’s status, indicating progress, maintenance, or decline; must ensure that a resident’s abilities in activities of daily living do not diminish unless circumstances of the resident’s clinical condition demonstrate that decline was unavoidable
  • Responsible to assure incident reports are filled out when needed, and properly done; assist DON with instruction when areas are incomplete or not accurately assessed
  • Document and review on 24 hr. report daily. Identify any pertinent issues and/or changes; assure that each shift follows through with needed documentation. Ensure physician and family are notified as per policy
  • Act as a resource for staff and provide on-going staff education
  • Follow up with staff when necessary to assure compliance to standards of
    documentation
