Schaumburg based Home Health Agency looking for visiting nurses that have clinical nursing knowledge and are directly responsible for planning and providing nursing care for a designated group of patients in accordance with the physician’s plan of care. This is accomplished by implementing, supervising, and coordinating services provided by the Agency and utilizing Agency and community resources. Currently hiring RN's *PER DIEM*

Responsibilities and Duties

  • Acts on referrals and takes physicians’ verbal orders as necessary. Channels referrals appropriately to Director of Nursing for assignment.
  • Makes assessment of patients during home visits and identifies patients’ needs, including need for additional health or ancillary services and admits patients for service when appropriate.
  • Assesses family and home environment and makes recommendations which will enhance patient care and promote safety.
  • Involves patient and/or family in developing an individualized care plan including necessary teaching with realistic goals.
  • Coordinates the home health care team and plans for patient care outcomes with other team members to provide appropriate, effective and efficient patient care.
  • Interprets admission, discharge and other policies of the Agency to patients, physicians, families, other health care providers, and community as needed.
  • (RN's) Provides written and verbal home health aide and LPN assignments and updates instructions for the aides and LPN’s as necessary.
  • Provides skilled nursing care and assessment to patients in the home environment according to the plan of care and promptly revises the plans as necessary in consultation with physicians and other team members as appropriate.
  • Explains to patient and family the diagnosis and the nature of the treatment consistent with the orders of the physician.
  • Provides progress notes to the patient's physician or podiatrist about patients under care when the patient's conditions change or there are deviations from the plan of care or at least every sixty days.
  • Administers medications and treatments as prescribed by the patient’s physician or podiatrist.
  • (RN's) Performs LPN Supervisory visits to evaluate the patients’ condition, plan of care and services provided by the LPN at least once every 4-6 visits, or more frequently if necessary.
  • (RN's) Conducts home health aide supervisory visits at least every two weeks and evaluates the aide’s competency, the assignment and patient’s continued need for aide services.
  • Assists in orienting and developing new staff when necessary.
  • Completes clinical notes and other required documentation appropriately and in timely manner.
  • Discusses necessary information to the Director of Nursing as instructed.
  • Maintains high standards of clinical care and serves as a clinical source.
  • Attends and participates in multi-disciplinary patient care conferences, in-service programs and staff meetings as assigned.
  • Works with patients, families and health care team members effectively to coordinate the Agency’s services, and to promote quality care and positive patient care outcomes.
  • Informs supervisory personnel of any patient incidents promptly and documents properly.
  • Serves as liaison between Agency, patients, and physicians.
  • Establishes and maintains good working relationships and open lines of communication with personnel from physician’s offices, area hospitals, and clinics, etc. to assure flow of information and referrals for service.
  • Supports Agency’s philosophy and adheres to the policies and procedures of the Agency.
  • Plans for discharge from the service.24.
  • Performs other duties assigned by the Director of Nursing

QUALIFICATIONS:

  • Must by a graduate from an accredited nursing program, and a holder of a current Illinois license as a Registered Nurse
  • At least one (1) year clinical nursing experience preferably in rehabilitation or medical/surgical nursing. Home health experience preferred.
  • Must have a good interpersonal, communication, and organizational skills necessary to complete job responsibilities.
  • Must have a vehicle and must be a holder of a current driver’s license and auto insurance.
  • Must be willing to make home visits and provide skilled nursing assessments and care in the home.
