Luther Park Community is seeking an very reliable and dependable Charge LPN for the Trinity Center at Luther Park – a 120-bed skilled nursing facility which specializes in Alzheimer’s care. We have full time openings on our 6a-2:30p shift. Includes working every other weekend.

*Must be able to pass criminal background check, provide references and pass post-offer/pre-employment physical, DRUG SCREEN, and TB test .

JOB SUMMARY:

 The Charge Nurse will be responsible for planning, organizing, and carrying out nursing care of residents. The job responsibilities include orienting, instructing, and supervising other personnel: CNA’s, CMA’s and resident care specialists. Carries out or assigns nursing duties to other personnel. The Charge Nurse complies with established policies and procedures and performs professional nursing duties within scope of licensure.

  1. Keeps DON/ADON and Evening shift supervisor informed of resident status and other pertinent matters. 
  2. Admit and orients new resident and involves family with orientation. Check physician’s orders, transfer forms and admission information. Reviews resident’s medical and social history. Completes admission and documentation forms. Make sure that residents supplies and medications are ordered and equipment is working properly. 
  3. Maintains proper documentation of facility records,i.e., narcotic records,medical chart forms and others, weekly skin assessment and updating wound pictures weekly as per the weekly skin assignment. Maintains daily, weekly, or monthly weight and vital signs, and informs physician/DON/ADON and On Call Manager of any change.

1. Graduate of an approved school of nursing for LPN or RN

2. Current nursing license in good standing in the state of Iowa

3. Current CPR certification preferred.

4. One year of experience in a geriatric setting preferred.

6. Knowledgeable with current changes in nursing practice.

7. Excellent Written and oral communication skills.
