ABOUT LLCHC 

Lower Lights Christian Health Center (LLCHC) transforms the overall health of Central Ohio, serving one individual at a time. We are focused on whole-person wellness, available to ALL in Central Ohio who need it, regardless of ability to pay! In 2019 alone, we served over 12,000 patients - with 40% being uninsured - and totaled 50,000 medical encounters!

Operating out of seven locations, we offer medical care (primary care, dental, vision, OB/GYN, telehealth), behavioral and psychiatric care, a low-cost pharmacy, nutritional assistance programs, and more. Working hours are Monday - Friday (varies by location 7:30-4:30, 8-5, 8:30-5:30) with occasional Saturday morning coverage.

JOB SUMMARY / DUTIES

The Community Health Worker is a frontline public health worker who is a trusted member of and/or has an in-depth understanding of the community served. The CHW serves as a liaison/link/intermediary between health/social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery. The CHW builds individual and community capacity and awareness by increasing health knowledge and self-sufficiency through a range of activities, such as outreach, community education, intensive case management, social support and advocacy. They assist the patient(s) in accessing appropriate resources and coordinates referrals as necessary, and they provide community and educational support, serving as a community liaison.

Primary Responsibilities will include the following: 

  • Provides outreach, engagement, and support for patients who experience overdose and other Substance Use Disorder-related needs in assigned Emergency Departments
  • Works closely with Mount Carmel Emergency Department staff and Social Care to identify, enroll, coordinate, and monitor patients in the program
  • Links patients to treatment, support, and resources in the community as needed
  • Demonstrates strong communication skills
  • Work with patients to plan and monitor care:
    • Assess unmet health and social needs
    • Develop care pathways
    • Monitor progress toward care pathway accomplishments and facilitate changes as needed
    • Follow up with patients, staff, and community resources to coordinate care

QUALIFICATIONS:

  • High School Diploma or general education degree (GED) required; Associate Degree preferred; *Peer Recovery Support certification is required *
  • Community Health Worker certification or ability to obtain within first year of employment.
  • CPR certified or ability to become certified within 6 months of hire
  • 1-3 years relevant community experience preferred
  • Must have and maintain a valid Ohio driver's license and dependable transportation
  • Strong communication skills and ability to serve as a community liaison between target populations, agencies, providers, and team
  • Competent in computer use and ability to utilize the internet and Microsoft programs 
  • Compassionate and competent in working with diverse cultural, religious, and socioeconomic groups
  • Prioritize work, engage in a variety of tasks simultaneously, and consistently meet deadlines
  • High degree of initiative, critical thinking, and independent judgment
