Live Well Hospice, LLC is Hiring a Remote Hospice LPN

Principle duties and accountabilities include, but are not limited to:

1. Participates as a member of the interdisciplinary team.

2. Contributes to the assessment of the health status of patients.

3. Participates in the development and modification of the Plan of Care.

4. Provides safe and effective nursing care consistent with the Nursing Practice Act.

5. Completes all required documentation.

6. Participates in on-call duties.

8. Participates in in-service training for staff and contracted facilities as assigned.

9. Complies with the state and federal regulatory requirements.

10. Complies with agency formularies and protocols.

Qualifications:

1. Licensed Practical Nurse with current state license.

2. Complies with acceptable professional standards and practice.

3. Demonstrates good communication.

4. Requires reliable transportation, current driver’s license and current auto insurance.

Job Type: Full-time

Pay: $25.00 - $28.50 per hour
