POSITION: Registered Nurse or Licensed Nurse Practitioner

DEPARTMENT: Lincoln Health Family Practice Clinics

PURPOSE:

Provides customer service and delivers nursing care to patients in a timely and professional manner.

Reports to: Clinic Administrator for administrative supervision and to the Physicians/Advanced Practice Providers in the Lincoln Health Family Practice Clinics for nursing supervision.

The Registered Nurse will work at a Family Practice Clinic that serves a largely rural/frontier population on the Eastern Plains.

DIMENSIONS:

The Registered Nurse will ensure the efficient and effective flow of patients during scheduled clinics through performing nursing duties. The employee will participate in the development of clinic objectives and participate in quality assessment and improvement processes and activities. The employee will maintain patient care supplies, equipment and the environment.

The Registered Nurse will protect the dignity, privacy and confidentiality of patients and their families, as well as co-workers and others.

Lincoln Health Family Practice Clinics support and encourage a multidisciplinary team

approach to whole person patient centered care with the integration of primary care and

behavioral health services.

ENVIRONMENT:

Lincoln Health is a 15-bed Critical Access Hospital co-located with 35-bed Long Term Care Unit. The facility employs 150-200 employees. Lincoln Health has 3 Family Practice Clinics, located in Hugo, Limon and Flagler. A Mobile Clinic that provides services all along the Eastern Corridor is also part of Lincoln Health Clinics.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

• Acts in a professional manner at all times.

• Works using the guidelines established from the Nurse Practice Act, Lincoln Health standards, policies and procedures and nursing judgment.

• Patient is approached in a kind, gentle and friendly manner. Respect for the patient’s dignity and privacy is consistently provided.

• Interventions are performed in a timely manner. Explanations for delays in responses are provided.

• Safety concerns are identified and appropriate actions are taken to maintain a safe environment.

• Rooms are neat and orderly

• Demonstrate behaviors consistent with facility mission, vision and values

• Maintains professional attire and appearance. Scrubs may be worn and are expected to be laundered and presentable.

• Acknowledges that all patients, family members, visitors, co-workers, employees of LCH and outside providers are customers.

• Provides care and interactions to customers with dignity and respect demonstrating quality customer service.

• Maintains confidentiality and patient rights regarding all patient information.

• Conducts self in a professional manner in compliance with facility policy.

Daily responsibilities

• Sort and file records on a daily basis to include all medical record information received from patient care staff, hospitals, physician offices and other outside sources.

• Obtain and record health histories

• Complete all documentation timely, efficiently and in its entirety

• Maintain health record and document all items required for Meaningful Use

• Verify all messages have been handled for the day

• Assisting with the front office scheduling, referrals, medical records, and assisting nursing staff in the Clinics.

• Maintains knowledge of equipment set-up, maintenance and use.

• Provides input in the formulation and evaluation of standards of care.

• Ensure tasks, labs, and diagnostics have been reviewed with patients and addressed as appropriate.

• Initiates emergency support measures (ie. CPR, protects patients from injury).

• Log off their work stations properly

• Provides excellent customer service to his/her team, patients, and public

• Maintains an organized and clean work area

• Emergency situations are recognized and appropriate action is instituted.

• All emergency equipment can be readily located and operated (emergency oxygen supply, drug box, fire extinguisher, etc.)

• Other patient care as assigned (patient recalls, reminders, telephone triage, injections, immunizations, etc. according to clinic policy and procedures).

• Attends all meetings required by CEO and Clinic Administrator

• Participates in clinic staff meetings

SUPERVISORY RESPONISIBILITIES:

The Registered Nurse provides supervision for all other nursing staff in all clinics. Works in conjunction with Clinic Administrator.

QUALIFICATION REQUIREMENTS:

To perform this job successfully, the Registered Nurse must be able to perform each essential duty satisfactorily. EDUCATION AND/OR EXPERIENCE:

Education:

• Graduation from an Associate’s or Bachelor’s Degree program in professional nursing.

Licensures:

• Current, unrestricted license to practice nursing in the State of Colorado.

• Certification in BLS (Basic Life Support) and ACLS (Advanced Cardiac Life Support); PALS recommended

Relevant Work Experience:

• Experience in providing nursing services in a rural area, primarily family practice clinics is recommended.

Knowledge, Skills, & Abilities:

• Basic understanding of computer skills: Excel, Word and EHR, as well as basic typing skills.

LANGUAGE SKILLS:

Ability to read, analyze and interpret financial reports and legal documents. Ability to respond to common inquiries or complains from patients, regulatory agencies or a member of the board. Ability to effectively present information to public groups and/or boards of directors.

REASONING ABILITY:

• Ability to define problems, collect data, establish facts and draw valid conclusions

• Participates in the identification of staff educational needs.

• Participates in the development of clinic objectives.

• Participates in the quality assessment and improvement process and activities.

OTHER SKILLS AND ABILITIES:

Ability to use a computer. Knowledge of basic computer functions and electronic health records. Basic knowledge of Microsoft Word and Microsoft Excel.
