Lilac Health Group LLC is Hiring a Social Services Director Near Royal Palm Beach, FL

Royal Palm Beach Health and Rehabilitation Center a 120 bed Skilled Nursing Facility located in Royal Palm Beach, Florida is currently seeking a Social Services Director. We are a growing facility in need of an energetic and talented professional to join our team.

As the Social Services Director (SSD) within a Skilled Nursing Facility, you will ensure that the Social Services department remains focused on the continuous efforts to develop and improve the overall well-being of residents. To be most successful in the role of Social Services Director, you should be a goal-oriented TEAM player who takes pride in leading by example.

Social Services Director Job Summary & Qualifications:

  • Collect data used in discharge plans, MDS and Care Plans
  • Assist new residents in adjusting to the facility.
  • Advocate for the residents and act as the facility Grievance Coordinator.
  • Coordinate and Lead staff meetings and other family and group meetings.
  • Minimum 3-5 years' experience in social work, preferably in an LTC, SNF, or hospital facility.
  • Ability to communicate at all levels of the organization, and work well within a TEAM environment
  • Customer service oriented with the ability to work well under pressure.
  • Ability to work with minimal supervision, take initiative and make independent decisions.
  • Approachable, flexible, highly adaptable to change
